患者男,76岁。因“胸闷气急5天,加重3小时”入院。入院时血压88~108/54~78 mmHg,心率98次/分,呼吸18~22次/分,血氧饱和度90%~93%,神志清,精神焦虑,两肺呼吸音低。胸部CT:两下肺动脉栓塞。双下肢深静脉造影:左侧髂静脉血栓形成。诊断:肺动脉栓塞,左侧髂静脉血栓形成。立即给予肺动脉血栓旋切联合抽栓+下腔静脉滤器治疗,选择OptEase滤器一枚(图1),患者回病房后给予左下肢局部溶栓治疗,尿激酶30万单位,q8h,经左侧足背静脉静滴,溶

Chest CT: two pulmonary embolism. Lower extremity deep venography: left iliac vein thrombosis. Diagnosis: Pulmonary embolism, left iliac vein thrombosis.
