[目的]探讨角蛋白19片段K19-2G2检测在肺癌中的临床意义。[方法]应用化学发光免疫分析法和电化学发光法检测345例肺癌、150例肺部良性疾病和500例健康体检者(对照组)血清K19-2G2和CYFRA21-1。[结果]肺癌组患者血清K19-2G2和CYFRA21-1浓度均显著高于肺部良性疾病和对照组(P<0.01);K19-2G2检测对肺癌的灵敏性和特异性分别为61.45%和97.69%均高于CYFRA21-1(P<0.05)。K19-2G2和CYFRA21-1在非小细胞肺癌的血清浓度显著高于小细胞肺癌(P均<0.05)。早期非小细胞肺癌和进展期非小细胞肺癌血清K19-2G2浓度和阳性率相近。[结论]K19-2G2对肺癌尤其是非小细胞肺癌的具有较高的灵敏性和特异性,可能有助于非小细胞肺癌早期诊断。
[Objective] To investigate the clinical significance of K19-2G2 detection of keratin 19 fragment in lung cancer. [Methods] The chemiluminescence immunoassay and chemiluminescence were used to detect the serum K19-2G2 and CYFRA21-1 in 345 cases of lung cancer, 150 cases of benign pulmonary diseases and 500 cases of healthy subjects (control group). [Results] The serum concentrations of K19-2G2 and CYFRA21-1 in lung cancer patients were significantly higher than those in benign lung diseases and controls (P <0.01). The sensitivity and specificity of K19-2G2 in lung cancer were 61.45% and 97.69 % Higher than CYFRA21-1 (P <0.05). Serum concentrations of K19-2G2 and CYFRA21-1 in non-small cell lung cancer were significantly higher than those in small cell lung cancer (all P <0.05). The serum concentration of K19-2G2 in early non-small cell lung cancer and advanced non-small cell lung cancer was similar to the positive rate. [Conclusion] K19-2G2 has high sensitivity and specificity for lung cancer, especially non-small cell lung cancer, which may contribute to the early diagnosis of non-small cell lung cancer.
