目的:运用斑点成像技术矩阵取样的方法在左心室前壁心肌额断面上,分析左室前壁心肌上25个感兴趣区(ROIs)的应变与应变率,观察左心室前壁缺血时左心室前壁额断面局部心肌功能的变化。方法:选择左心室前壁缺血(缺血组)患者及无左心室前壁缺血(对照组)的患者各20例,在左心室前壁心肌额断面超声采集25个ROIs的斑点追踪所获的收缩期应变(ε)、收缩期应变率(SRS)、舒张早期应变率(SRE)及舒张晚期应变率(SRA),比较两组患者应变及应变率的差异,并以对照组SRS、SRE及SRA的95%医学参考值范围作为阈值参考,计算缺血组异常ROIs的百分率(PAR)。结果:25个ROIs的斑点追踪测得缺血组SRS与SRE均小于对照组(P<0.05);以对照组应变率的测量值制定95%医学参考值范围为基准参数,观察到缺血组的左心室前壁心肌SRS与SRE低于对照组基准水平18.66%~25.35%。结论:应用左心室前壁心肌额断切面以矩阵采样的方式,采集25个ROIs的斑点追踪局部左室功能参数,通过心肌异常位点的检出率(PAR)能够检测出左心室前壁心肌缺血患者的局部心肌功能异常。
OBJECTIVE: To analyze the strain and strain rate of 25 regions of interest (ROIs) on the anterior myocardium of the left ventricular wall using the speckle imaging technique matrix sampling method on left anterior myocardial wall in front of the myocardium. Methods: 20 patients with anterior anterior wall ischemia (ischemic group) and without anterior left ventricular ischemic (control group) were selected. Twenty-five ROIs spots were collected by ultrasonography in the anterior wall of left ventricular myocardium The systolic strain (ε), systolic strain rate (SRS), early diastolic strain (SRE) and late diastolic strain rate (SRA) were obtained. The differences of strain and strain rate between the two groups were compared. SRE and SRA 95% of the medical reference value range as a threshold reference to calculate the percentage of ischemic abnormal ROIs (PAR). Results: The SRS and SRE of 25 ROIs were less than those of the control group (P <0.05). 95% of the reference range of the medical reference value was made based on the measurement of the strain rate in the control group.
