目的了解海南省影响乙肝报告质量的主要因素,为制定乙肝控制策略提供科学依据。方法采取随机整群分层抽样方法选取调查样本,根据《海南省2006年医疗机构乙肝病例报告调查方案》进行调查。结果共调查9个市县(区)44家医疗机构,总计查录到乙肝初诊登记病例1369例,其中已经网络报告的有595例,经现场核实为确诊病例的有421例,网络报告病例中符合调查诊断标准的确诊病例有347例,将病原携带者报为确诊病例的有124例,误报率20.84%,将疑似病例报为确诊病例的有101例,误报率16.97%,总误报率37.82%;2006年调查地区乙肝报告发病率为90.76/10万,估计实际发病率为52.93/10万,报告准确率仅为58.32%,实际估计发病率明显低于报告发病率,低于41.68%。结论我省乙肝病例报告误报率高,导致误报的主要原因是将病原携带者和疑似病例作为乙肝确诊病例进行报告,提示对医务人员加强传染病诊断标准的培训,确保乙肝病例报告质量是今后工作的重点。
Objective To understand the main factors affecting the quality of hepatitis B reporting in Hainan Province and provide a scientific basis for formulating hepatitis B control strategies. Methods Random stratified stratified sampling method was used to select the survey samples, according to “Hainan Province in 2006 medical institutions hepatitis B case report survey program” to investigate. Results A total of 44 medical institutions in 9 cities and counties (districts) were investigated. A total of 1369 cases of newly diagnosed cases of hepatitis B were investigated. Among them, 595 cases were reported by the network, 421 cases were confirmed by on-site diagnosis, There were 347 confirmed cases that met the diagnostic criteria of diagnosis. There were 124 cases of pathogen carriers reported as confirmed cases, the false positive rate was 20.84%. The number of suspected cases was confirmed as 101 cases, the false positive rate was 16.97% Reported 37.82%; in 2006, the incidence of hepatitis B in the surveyed area was 90.76 / 100000, the estimated actual incidence was 52.93 / 100000, the report accuracy was only 58.32%, the actual estimated incidence was significantly lower than the reported incidence, lower than 41.68%. Conclusion The reported false positive rate of hepatitis B cases in our province is high. The main cause of false positives is the report of pathogen carriers and suspected cases as confirmed cases of hepatitis B, which prompts the training of medical personnel to strengthen the diagnostic criteria of infectious diseases and ensure the quality of hepatitis B case reports The focus of future work.
