目的探讨岛状皮瓣缺血再灌注损伤中血液流变学的变化及川芎嗪的保护性作用。方法将112只大鼠随机分为假手术组(SO组)、对照组(IR组)、低分子右旋糖酐组(低右组)和川芎嗪组(TMP组),每组再随机分为2 h组、4 h组、6 h组、8 h组。在术前30 min分别给予生理盐水(SO组、IR组)、低分子右旋糖酐、川芎嗪注射液腹腔注射,一次性给药。术后检测各组大鼠血液中丙二醛(MDA)含量和超氧化物歧化酶(SOD)活性及血液流变学指标。结果除血浆黏度外,TMP组、低右组的6 h、8 h分别和IR组、SO组的6 h、8 h比较,血液流变学差异有统计学意义(P<0.05),TMP组和低右组各自时间点的比较血流变差异无统计学意义(P>0.05)。SOD:IR组内6 h、8 h与2 h、4 h比较,川芎嗪组和低右组的6 h、8 h与IR组的6 h、8 h组间比较,差异有统计学意义(P<0.05);MDA:IR组内8 h与2 h、4 h比较,川芎嗪组和低右组的6 h、8 h与IR组的6 h、8 h比较,差异有统计学意义(P<0.05)。结论川芎嗪注射液能够防治游离皮瓣缺血再灌注损伤。
Objective To investigate the changes of hemorheology in ischemia-reperfusion injury of island flaps and the protective effect of ligustrazine. Methods One hundred and twelve rats were randomly divided into sham operation group (SO group), control group (IR group), low molecular dextran group (low right group) and ligustrazine group (TMP group) Group, 4 h group, 6 h group, 8 h group. At 30 min before operation, saline (SO group, IR group), low molecular weight dextran and ligustrazine injection were given intraperitoneally for one-time administration. Postoperative blood levels of malondialdehyde (MDA) and superoxide dismutase (SOD) and blood rheology were detected in each group. Results In addition to plasma viscosity, TMP group, low right group at 6 h, 8 h and IR group, SO group 6 h, 8 h, hemorheological differences were statistically significant (P <0.05), TMP group There was no significant difference in the blood rheology between the right and left groups at each time point (P> 0.05). SOD: Compared with 6 h, 8 h and 2 h, 4 h in IR group, 6 h and 8 h in ligustrazine group and low right group and 6 h and 8 h group in IR group, the differences were statistically significant ( P <0.05); MDA: Compared with 2 h, 4 h in 8 h and 2 h, 4 h in IR group, there was significant difference between 6 h and 8 h in ligustrazine group and low right group and 6 h and 8 h in IR group P <0.05). Conclusion Ligustrazine injection can prevent ischemia-reperfusion injury of free flap.
