目的探讨安氏Ⅱ1类错拔牙矫治过程中,上颌尖牙远中移位对前后牙位置的影响。方法选择2011年1月至2012年12月我院收治的安氏Ⅱ1类错患者17例,前牙Ⅱ覆8例,Ⅲ覆9例。所有患者的矫治均需拔除四个第一双尖牙,并采用口外弓加强上颌磨牙支抗配合方丝弓矫治技术进行治疗。治疗前及以间断力后移上颌尖牙到与第二双尖牙接触时,进行X线头影测量及统计分析。结果上颌磨牙前移了0.9 mm,比下颌磨牙整体前移少约2.1 mm;上切牙舌侧斜移动了4.2 mm;下切牙舌向移动了1.6 mm,比上颌磨牙前移少1.0 mm。结论控制上颌磨牙支抗并增大弓丝的滑动,可使上颌切牙在未主动内收前舌向移动;减少下颌尖牙的后移则有利于下颌磨牙的前移。
Objective To investigate the effect of distal maxillary canine shift on the position of anterior and posterior teeth during the correction of Class Ⅱ division 1 malocclusion. Methods From January 2011 to December 2012, 17 cases of Class Ⅱ 1 malocclusion were treated in our hospital, including 8 cases of anterior teeth Ⅱ and 9 cases of Ⅲ. All patients were required to remove the four first pairs of canine teeth, and the use of extraoral arch to enhance maxillary molar support surgery arch wire correction techniques for treatment. Before treatment and after intermittent force to move the maxillary canine to contact with the second premolar, the X-cephalometry and statistical analysis. Results The maxillary molars moved forward by 0.9 mm, which was 2.1 mm less than the total displacement of the mandibular molars. The lingual incision moved 4.2 mm diagonally and the lower incisor moved 1.6 mm more than the maxillary molars. Conclusion Controlling maxillary molar support and increasing slippage of the arch wire can move the maxillary incisor in the involuntary adduction anterior lingual movement and reduce the posterior mandibular molar posterior shift in favor of mandibular molar advancement.
