目的应用三维斑点追踪成像评估射血分数正常的OSAS患者左心室功能。方法选取轻、中、重度OSAS患者和健康者各30例。应用三维超声心动图获取三维纵向、径向、圆周收缩期整体应变(GS)、收缩期整体应变率(GSR_S)、舒张早期整体应变率(GSR_E)、舒张晚期整体应变率(GSR_A)、收缩期达峰时间最大差值TTP-max等参数。结果中、重度OSAS组三维纵向收缩功能明显减弱(P<0.05);重度OSAS组三维圆周收缩功能明显减弱(P<0.05);轻、中度OSAS组三维径向收缩功能增强,重度OSAS组减低。三维纵向、径向、圆周舒张早期应变减低,舒张晚期应变增大。结论射血分数正常OSAS患者存在心肌受损可能,三维斑点追踪成像在评估心肌功能上具有重要意义。
Objective To evaluate left ventricular function in OSAS patients with normal ejection fraction by three-dimensional speckle tracking imaging. Methods Thirty patients with mild, moderate and severe OSAS and 30 healthy individuals were selected. Three-dimensional echocardiography was used to obtain the global systolic strain (GS), global systolic strain (GSR_S), global early diastolic strain (GSR_E), global systolic strain (GSR_A), systolic Peak time difference TTP-max and other parameters. Results In the severe OSAS group, the three-dimensional longitudinal systolic function was significantly attenuated (P <0.05), and the three-dimensional systolic function was significantly decreased in severe OSAS group (P <0.05) . Three-dimensional vertical, radial, circumferential diastolic early strain decreased, late diastolic strain increased. Conclusion Myocardial damage may exist in OSAS patients with normal ejection fraction. Three-dimensional speckle tracking imaging is of great significance in evaluating myocardial function.
