目的 :了解抑凋亡基因 bcl- 2、抑癌基因 p5 3和癌基因 p2 1基因蛋白在肝细胞癌 (HCC)中的表达状况及意义。方法 :用 bcl- 2、p5 3和 p2 1基因蛋白抗体对 5 5例肝细胞癌合并结节性肝硬变手术标本进行 S- P免疫组化检测 ,分析基因蛋白表达率与病理分级和预后的关系。结果 :bcl- 2基因蛋白在 HCC和结节性肝硬变中的表达率分别为 72 .7% (4 0 /5 5 )和 36 .4% (2 0 /5 5 ) ,两者存在差异 (P<0 .0 5 ) ;p5 3蛋白在 HCC中表达率为 5 4.5 % (30 /5 5 ) ,而肝硬变组织均呈阴性表达 ,两者存在显著差异 (P<0 .0 1) ;p2 1蛋白在 HCC和结节性肝硬变中的表达率分别为 6 3.6 % (35 /5 5 )和 38.2 %(2 1/5 5 ) ,两者无差异 (P>0 .0 5 ) ;而 bcl- 2、p5 3和 p2 1的表达率和表达强度均与 HCC的病理分级无关 (P>0 .0 5 )。另外 ,在随访 2 9例 HCC患者中发现生存率短组 (6个月至 1年 )的 16例中 ,bcl- 2、p5 3和 p2 1的共同表达率为 5 0 % (8/16 )。研究结果提示 :HCC的发生、发展与 bcl- 2、p5 3和 p2 1蛋白的异常表达或突变有关 ;bcl- 2、p5 3和 p2 1之间可能存在相互协同作用 ,对 HCC的预后有一定的影响 ;bcl- 2、p5 3和 p2 1蛋白过度表达反应了 HCC癌细胞的分化程度及增殖情况 ,因而可作为 HCC诊断和预后的参考指标之?
Objective: To investigate the expression and significance of the suppressor gene bcl-2, tumor suppressor gene p53 and oncogene p21 gene in hepatocellular carcinoma (HCC). METHODS: S-P immunohistochemistry was performed on 55 specimens of hepatocellular carcinoma with nodular liver cirrhosis by bcl-2, p53, and p21 gene antibody. The expression of gene protein, pathological grade and prognosis were analyzed. Relationship. Results: The expression rates of bcl-2 gene protein in HCC and nodular liver cirrhosis were 72.7% (4 0 /5 5) and 36.4% (2 0 /5 5), respectively. (P<0.05); The expression rate of p53 protein in HCC was 54.5 % (30 /5 5), whereas the cirrhosis was negative. There was a significant difference between the two (P<0. 0 1). The expression rates of p21 protein in HCC and nodular liver cirrhosis were 6 3.6 % (35 /5 5) and 38.2 % (2 1/5 5), respectively. There was no difference between the two (P>0. 5) The expression and intensity of bcl-2, p53 and p21 were not associated with the pathological grade of HCC (P>0.05). In addition, the co-expression rate of bcl-2, p53, and p21 was found to be 50% (8/16) in 16 patients with short-term survival (6 months to 1 year) in a follow-up of 29 HCC patients. . The results suggest that the occurrence and development of HCC are related to the abnormal expression or mutation of bcl-2, p53, and p21 proteins, and there may be synergy between bcl-2, p53, and p21, and have a certain prognosis for HCC. The effect of over-expression of bcl-2, p53, and p21 proteins reflects the differentiation and proliferation of HCC cancer cells, which can be used as a reference marker for the diagnosis and prognosis of HCC.
