目的 比较经颅多普勒 (TCD)与磁共振血管成像 (MRA)对缺血性脑血管病颅内动脉狭窄或闭塞诊断的准确性。方法 回顾分析 5 0例缺血性脑血管病患者的TCD与MRA检测结果 ,全部病例均有脑血管造影 (DSA)对比验证。结果 TCD对缺血性脑血管病颅内动脉狭窄或闭塞诊断的特异性为 98.6 % ,敏感性为 83.9% ;MRA分别为 92 .6 %和 79.7%。TCD假阳性率为 10 .3% ,假阴性率为 2 .5 % ;MRA分别为 35 .7%和 3.5 %。结论 在诊断缺血性脑血管病颅内动脉狭窄或闭塞方面 ,TCD与MRA均有很好的可靠性 ,但TCD较MRA有更高的准确性。
Objective To compare the accuracy of transcranial Doppler (TCD) and magnetic resonance angiography (MRA) in the diagnosis of intracranial arterial stenosis or occlusion in ischemic cerebrovascular disease. Methods The TCD and MRA findings of 50 patients with ischemic cerebrovascular disease were retrospectively analyzed. All cases were confirmed by cerebral angiography (DSA). Results The specificity of TCD for diagnosing intracranial arterial stenosis and occlusion in ischemic cerebrovascular disease was 98.6% and the sensitivity was 83.9%. The MRA was 92.6% and 79.7% respectively. TCD false positive rate was 10.3%, false negative rate was 2.5%; MRA were 35.7% and 3.5%. Conclusion TCD and MRA have good reliability in diagnosing intracranial arterial stenosis or occlusion in ischemic cerebrovascular disease, but TCD is more accurate than MRA.
