位置性眼震系指当头处于某一位置时出现并在这一头位持续存在的眼震(Bárány,1913)。Cawthorne(1961)称这种眼震为中枢性位置性眼震,并指出其与颅后窝病变有关。本文报告4例多发性硬化症出现中枢型位置性眼震,3例证实病变在第四脑室,1例未肯定。 3名22~28岁的年青妇女,就诊前即有发作性眩晕及轻度恶心,仰卧和侧卧时加重。临床检查除不稳、睁眼位置性眼震外无其他发现;只要维持临界体位,眼震即持续,眩晕、恶心加重。其中一名病人首次犯病后五天发作频繁,二个月后症状和眼震消失,闭眼时可记录到右侧位位置
Postive nystagmus refers to nystagmus that occurs when the head is in a certain position and persists at this head (Bárány, 1913). Cawthorne (1961) described this nystagmus as a central positional nystagmus and pointed out that it is associated with lesions in the posterior fossa of the skull. This article reports 4 cases of multiple sclerosis central-type nystagmus, 3 confirmed lesions in the fourth ventricle, 1 was not confirmed. Three young women aged 22 to 28 years old had episodic dizziness and mild nausea before going to the clinic, and were aggravated when supine and lateral. In addition to instability clinical examination, open eyes no other place outside of the nystagmus; as long as the maintenance of critical position, nystagmus that is sustained, dizziness, nausea increased. One of the patients had frequent seizures for five days after the first illness and two months later the symptoms and nystagmus disappeared.
