目的探讨三维CT(3DCT)影像学分析在小儿血管瘤诊断和治疗中的价值。方法根据不同三维成像,分析20例患儿血管瘤的血液供应及其与血管瘤类型的关系。结果20例中,呈现斑片状影3例、絮状影3例、团块影6例、蚯蚓状影4例、混合影4例。根据3DCT影像学分析定位血管瘤的供血及回流血管,行单纯结扎滋养血管或手术切除后皮瓣转移修复创面18例,均治愈;未行手术2例,观察1年半后消退。结论3DCT影像能提高对血管瘤血供的判断,指导小儿血管瘤的临床诊治。
Objective To investigate the value of three-dimensional CT (3DCT) imaging in the diagnosis and treatment of pediatric hemangiomas. Methods According to different three-dimensional imaging, the blood supply of hemangiomas in 20 cases and their relationship with hemangiomas were analyzed. Results In 20 cases, 3 cases were patchy, 3 cases were flocculent, 6 cases were bloomed, 4 cases were earthworm shadow and 4 cases were mixed shadow. According to 3DCT imaging analysis of hemangiomas and blood vessels, 18 cases of wounds were repaired by simple ligation of nourishing blood vessels or surgical excision. All cases were cured. No operation was observed in 2 cases and disappeared after 1 year and a half. Conclusion 3DCT images can improve the blood supply of hemangiomas and guide the diagnosis and treatment of pediatric hemangiomas.
