目的探讨胃神经内分泌细胞(NE)在十二指肠溃疡中的变化。方法对8例正常胃及6例十二指肠溃疡(DU)胃粘膜的 NE 细胞应用透射电镜进行分类,并应用超微图像分析仪对 NE 细胞各种参数进行定量测量。结果发现正常胃窦及胃体 NE 细胞密度分别为1.6%及0.9%,DU 为1.3%及0.8%(P>0.05)。正常及 DU 中胃窦 G 细胞分别占65%及46%(P>0.05),D 细胞分别占9.5%及3%(P<0.05)。胃体 ECL 细胞分别占30%及49%(P<0.05);D 细胞占22%及2%(P<0.01);EC 细胞分别占7%及4%(P<0.05)。D细胞的单位计数明显降低(P<0.01),胞浆比例增大(P<0.05)。结论十二指肠溃疡患者胃泌素升高可能与 D细胞减少,分泌生长抑素降低,对 G 细胞分泌胃泌素的抑制作用下降有关。
Objective To investigate the changes of gastric neuroendocrine cells (NE) in duodenal ulcer. Methods Eight kinds of normal gastric tissue and 6 cases of duodenal ulcer (DU) gastric mucosa of the NE cells were classified by transmission electron microscopy, and the use of a micro-image analyzer for quantitative determination of various parameters of NE cells. The results showed that the density of NE cells in normal gastric and gastric tissues were 1.6% and 0.9% respectively, and DU was 1.3% and 0.8% respectively (P> 0.05). Normal and DU gastric antrum G cells accounted for 65% and 46% (P> 0.05), D cells accounted for 9.5% and 3% (P <0.05). The percentage of ECL cells in gastric body was 30% and 49% respectively (P <0.05). D cells accounted for 22% and 2% respectively (P <0.01). EC cells accounted for 7% and 4% respectively (P <0.05). The number of D cells decreased significantly (P <0.01), and the proportion of cytoplasm increased (P <0.05). Conclusions The increase of gastrin in patients with duodenal ulcer may be related to the decrease of D cells, the decrease of somatostatin secretion and the decrease of the secretion of gastrin by G cells.
