目的调查乳腺癌术后辅助治疗患者的心理现状及其影响因素,为更好地对乳腺癌患者进行心理护理、提高生活质量提供依据。方法采用标准心理痛苦温度计(distress thermometer,DT)对四川大学华西医院肿瘤中心行乳腺癌术后辅助治疗患者的心理状况进行问卷调查。采用χ2检验分析乳腺癌患者心理痛苦(DT≥4分)与人口统计学特征(分类变量)、临床特征及问题列表中各条目的关系,t检验分析乳腺癌患者心理痛苦(DT≥4分)与年龄(连续变量)的关系。结果共发放问卷207份,回收有效问卷196份(94.7%)。107例(54.6%)患者DT≥4分,22例(11.2%)患者DT≥7分。在DT的问题列表上,乳腺癌患者描述的各类问题频率有16个条目超过30%。影响因素分析:与问题列表中4个问题版块39个条目中的14个条目相关(P<0.05)。结论超过一半的乳腺癌术后辅助治疗患者存在心理痛苦。乳腺癌术后给女性患者造成的心理痛苦来源于现实、家庭、情绪、身体等多个方面。应加强乳腺癌术后患者的心理护理、及时用DT筛查识别患者的心理痛苦并帮助患者解决与心理痛苦有关的问题,提高乳腺癌患者的生活质量。
Objective To investigate the psychological status of patients with postoperative adjuvant breast cancer and its influencing factors in order to provide a basis for better psychological care and quality of life in patients with breast cancer. Methods A standard psychological distress thermometer (DT) was used to investigate the psychological status of patients with postoperative adjuvant breast cancer treated with the West China Hospital Cancer Center in Sichuan University. The χ2 test was used to analyze the relationship between psychological pain (DT≥4) and demographic characteristics (categorical variables), clinical features and the items in the questionnaire, and t test was used to analyze the psychological pain (DT≥4) in patients with breast cancer. Relationship with age (continuous variables). Results A total of 207 questionnaires were distributed and 196 valid questionnaires (94.7%) were recovered. 107 patients (54.6%) had DT≥4 points and 22 patients (11.2%) had DT≥7 points. On the DT’s list of questions, breast cancer patients describe more than 30% of the 16 questions in all categories. Impact Factor Analysis: Associated with 14 of the 39 entries in the 4 questions section of the list of issues (P <0.05). Conclusions More than half of postoperative adjuvant breast cancer patients have psychological distress. The psychological pain caused by postoperative breast cancer to female patients comes from many aspects such as reality, family, emotion and body. Should strengthen the psychological care of postoperative patients with breast cancer, DT screening in a timely manner to identify patients with psychological pain and to help patients with psychological pain related problems and improve the quality of life of breast cancer patients.
