目的探讨血管内皮生长因子(VEGF)在三阴性乳腺癌(TNBC)中的表达及其预后价值。方法选择2000年6月-2014年10月中国石油天然气集团公司中心医院病理科的TNBC组织石蜡标本(TNBC组)和非三阴性乳腺癌组织石蜡标本(非TNBC组)为研究对象,各50例,另选择正常乳腺组织30例为正常对照组。采用SP免疫组织化学法检测VEGF蛋白在三组中的表达,并分析VEGF的表达与TNBC临床病理因素的关系。结果 VEGF在TNBC组、非TNBC组及正常乳腺组织中的阳性表达率分别为88.0%(44/50)、50.0%(25/50)、26.7%(8/30),TNBC组和非TNBC组中VEGF阳性表达率均明显高于正常对照组,而TNBC组中VEGF阳性表达率明显高于非TNBC组(χ2=8.17,P<0.05)。TNM分期(χ2=8.04,P<0.05)、组织学分级(χ2=7.39,P<0.05)、腋窝淋巴结转移(χ2=6.18,P<0.05)与VEGF表达的相关性差异有统计学意义。Logistic回归多因素分析结果显示腋窝淋巴结转移、TNM分期是VEGF表达的影响因素。结论 VEGF过度表达可能在TNBC的恶变、进展等过程中起更重要的作用,且其表达与TNBC的淋巴结转移、侵袭性强和容易远处转移的生物学行为密切相关。
Objective To investigate the expression and prognostic value of vascular endothelial growth factor (VEGF) in triple negative breast cancer (TNBC). Methods TNBC tissue paraffin specimens (TNBC group) and non-triple negative breast cancer tissue paraffin specimens (non-TNBC group) from June 2000 to October 2014 in the Central Hospital of China National Petroleum Corporation were selected as study objects, 50 cases each , Another 30 cases of normal breast tissue as normal control group. SP immunohistochemistry was used to detect the expression of VEGF protein in three groups, and to analyze the relationship between the expression of VEGF and TNBC clinical pathological factors. Results The positive rates of VEGF expression in TNBC group, non-TNBC group and normal breast tissue were 88.0% (44/50), 50.0% (25/50) and 26.7% (8/30) respectively. TNBC group and non-TNBC group The positive rate of VEGF expression in TNBC group was significantly higher than that in normal control group, while the positive rate of VEGF in TNBC group was significantly higher than that in non-TNBC group (χ2 = 8.17, P <0.05). The correlation between TNM stage (χ2 = 8.04, P <0.05), histological grade (χ2 = 7.39, P <0.05), axillary lymph node metastasis (χ2 = 6.18, P <0.05) and VEGF expression was statistically significant. Logistic regression multivariate analysis showed that axillary lymph node metastasis, TNM staging is the influencing factor of VEGF expression. Conclusion Overexpression of VEGF may play a more important role in the malignant transformation and progression of TNBC. The expression of VEGF is closely related to the biological behavior of lymph node metastasis, invasive aggressiveness and distant metastasis in TNBC.
