患者,女性,23岁,未婚。因外阴部有一进行性增大的赘生物4年而就诊。检查;全身情况良好,于右侧大阴唇见一带蒂赘生物约12×10×10 cm,质地柔软,表面皮肤皱缩。在赘生物下极有一溃疡面约6×7 cm。蒂细长约5 cm,直径1.5 cm。拟诊为乳头状瘤,在局麻下行肿物切除术。病理检查:带蒂肿物大小14×10×8.5 cm,重0.5 kg。表面被覆皮肤,肿物远端部分区域糜烂,切面灰白色,质地软而韧,实性,网织状。镜下见表面为扁平鳞状上皮,部分呈低乳头状增生。真皮由疏松的胶原纤维构成,间质含有较丰富的毛细血管。病理诊断:蒂袋状软纤维瘤。皮肤软纤维瘤又称软垂疣或皮赘,大小一般为1～3 cm,临床上巨大者罕见。软纤维瘤可分为丝状软纤维瘤和蒂袋状软纤维瘤两种。前者常呈多发性,好发于颈部和腋窝,外形因呈丝状或乳头状
