目的:分析维持性血液透析患者血清肌钙蛋白(cTnT)及B型钠尿肽(BNP)数值波动的意义。方法:选取2016年2月-2017年5月本院收治的维持性血液透析患者110例作为试验组,并同时选取同期在本院体检中心检查的健康人群110例作为对照组,对比两组患者血清肌钙蛋白T和B型钠尿肽水平的变化及意义。结果:两组收缩压、TG、HDL-C、LDL-C、cTnT、BNP、CRP、血肌酐、尿素氮、血尿酸、空腹血糖、餐后2 h血糖水平比较,差异均有统计学意义(P<0.05)。BNP阳性组和BNP阴性组、cTnT阳性组和cTnT阴性组在临床表现、超声心动图异常情况比较,差异均有统计学意义(P<0.05);BNP阳性组和BNP阴性组、cTnT阳性组和cTnT阴性组在心电图异常情况比较,差异无统计学意义(P>0.05);cTnT阳性组的死亡率、糖尿病发生率均高于cTnT阴性组,BNP阳性组的死亡率、糖尿病发生率均高于BNP阴性组,差异均有统计学意义(P<0.05)。结论:血清肌钙蛋白T与B型钠尿肽水平对于早期评估维持性血透患者心血管并发症意义重大,值得临床推广应用。
Objective: To analyze the significance of serum cTnT and BNP values in maintenance hemodialysis patients. Methods: One hundred and ten patients with maintenance hemodialysis who were treated in our hospital from February 2016 to May 2017 were selected as the experimental group, and 110 healthy subjects who were examined in our hospital during the same period were selected as the control group. BNP positive group and BNP negative group, cTnT positive group and cTnT negative group in clinical manifestations, echocardiography abnormalities, the difference was statistically significant (P <0.05); BNP positive group and BNP negative group, cTnT positive group and There was no significant difference in ECG abnormalities between cTnT negative group and cTnT positive group (P> 0.05). The mortality of cTnT positive group and the incidence of diabetes were higher than that of cTnT negative group, and the death rate and the incidence of diabetes were higher in BNP positive group than in cTnT negative group BNP negative group, the difference was statistically significant (P <0.05). Conclusion: Serum troponin T and B-type natriuretic peptide levels are of great significance for the early assessment of cardiovascular complications in maintenance hemodialysis patients, and are worthy of clinical application.
