目的探讨血清糖蛋白抗原125(CA125)对子宫内膜癌患者的诊疗价值。方法随机选取2013年1月至2015年12月入本院行子宫内膜癌手术治疗的患者347例,根据临床分期分为三组:早期组(Ⅰ期)、中期组(Ⅱ期、Ⅲ期)、晚期组(Ⅳ期);提前一周对所有患者随机访问并取静脉血,实验室采用国际癌症协会推荐的免疫发光法进行检测血清CA125浓度;检测其与病理分期、病理分级和组织学类型的关系。结果不同病理分期的CA125阳性者间的阳性率差异有统计学意义(P<0.05),手术病理分期越晚,子宫内膜癌患者的阳性率越高;不同肌层浸润深度患者的CA125阳性率差异有统计学意义(P<0.05),肌层浸润深度>1/2与<1/2的阳性率分别为68.2%、45.2%,CA125的阳性率随肌肤浸润深度增加;病理分期和组织学类型可以影响子宫内膜癌患者血清CA125阳性率,其差异有统计学意义(P<0.05)。Logistic回归分析显示,血清CA125阳性率与病理分级、组织学类型、肌层浸润深度和临床分期均有相关性(P<0.05)。结论患者手术前检测血清CA125对判定其临床分期、病理分级和组织学的类型有重要意义,对判断子宫内膜癌患者的病情有帮助。
Objective To investigate the value of serum glycoprotein antigen 125 (CA125) in diagnosis and treatment of patients with endometrial cancer. Methods A total of 347 patients with endometrial cancer who underwent surgery in our hospital from January 2013 to December 2015 were randomly divided into three groups according to clinical stage: early stage (stage Ⅰ), middle stage (stage Ⅱ, stage Ⅲ) ), Advanced group (Ⅳ); one week in advance, all patients were randomized to visit and take venous blood, the laboratory using the International Association of Cancer Immunoassay for the detection of serum CA125 concentration; detection and pathological staging, pathological grade and histological type Relationship. Results The positive rates of CA125 in different pathological stages were significantly different (P <0.05). The later the pathological stage was, the higher the positive rate of CA125 was. The positive rates of CA125 in different depths of myometrial invasion (P <0.05). The positive rates of myometrial invasion depth> 1/2 and <1/2 were 68.2% and 45.2% respectively. The positive rate of CA125 increased with the depth of skin invasion. The pathological stage and histology Type can affect the serum CA125 positive rate in patients with endometrial cancer, the difference was statistically significant (P <0.05). Logistic regression analysis showed that the positive rate of serum CA125 was correlated with pathological grade, histological type, depth of myometrial invasion and clinical stage (P <0.05). Conclusions The preoperative detection of serum CA125 in patients with clinical staging, pathological grading and histological types of significance, to determine the condition of patients with endometrial cancer is helpful.
