目的:探讨不典型少突胶质细胞肿瘤(OG)的影像表现,提高诊断水平。方法:回顾分析12例经病理证实不典型OG的临床CT和MRI资料,探讨其影像特征。结果:年龄范围:23~60岁,平均41.5岁,其中40岁以上10例(83.3%)。7例表浅,5例深在,仅1例多发,共13个病灶。9例呈实性,MRI平扫呈稍长T1稍长T2信号,边缘欠清,其中1例信号均匀,增强后未见强化,8例信号不均,内见坏死、出血,增强后,7例呈斑片状轻-中度强化,1例呈“花环状”强化,且DWI明显弥散受限。3例呈完全囊性,囊液呈长T1长T2信号,壁较厚,呈等T1稍长T2信号,边缘不清,增强后均未见强化,1例DWI序列囊壁不均匀弥散受限。3例CT片上可见钙化,2例瘤周见大片水肿,2例跨中线,1例向深部浸润、侵犯邻近基底节区及丘脑。结论:不典型OG包括:中老年人、多发、位置深在、瘤周大片水肿、跨中线生长、弥漫深部浸润、增强后环形强化、DWI明显弥散受限、完全囊性,其影像表现仍具有一定特征,综合分析可提高诊断准确率。
Objective: To investigate the imaging findings of atypical oligodendrocyte tumor (OG) and to improve the diagnostic level. Methods: Clinical CT and MRI data of 12 cases with atypical OG confirmed by pathology were retrospectively analyzed to investigate the imaging features. Results: The age range was 23-60 years with an average of 41.5 years, of which 10 (40.3%) were over the age of 40. 7 cases superficial, 5 cases deep, only 1 case multiple, a total of 13 lesions. 9 cases were solid, MRI scan showed a slightly longer T1 slightly longer T2 signal, the edge of the lack of clear, of which 1 case of uniform signal, no enhancement after enhancement, 8 cases of signal unevenness, internal necrosis, bleeding, enhanced, 7 Cases showed a patchy light - moderate enhancement, 1 case was “rosette ” enhanced, and DWI significantly limited diffusion. 3 cases were completely cystic, cystic fluid showed long T1 long T2 signal, the wall was thick, showing T1 slightly longer T2 signal, the edge was unclear, no enhancement after enhancement, 1 case of DWI sequence uneven dispersion of the wall of the wall . Three cases showed calcification on CT, two cases had large edema in the tumor, two cases crossed the midline, and one case infiltrated into the deep infiltration of adjacent basal ganglia and thalamus. CONCLUSIONS: Atypical OG include middle-aged and elderly patients with multiple, deep location, large peritumoral edema, transmural midline growth, diffuse deep infiltration, enhanced annular enhancement, limited diffusion of DWI, complete cystic appearance, A certain feature, a comprehensive analysis can improve the diagnostic accuracy.
