目的 探讨低位直肠癌超低位前切除的适应证。方法 结合文献分析 3家医院 1994年 1月～ 2 0 0 0年 12月间超低位前切除治疗低位直肠癌病人 2 6例。结果 5年生存率 (85 .4±8.1) % (寿命表法 )。总局部复发率 15 .4% ,中分化腺癌复发率 2 5 % (2 /8) ,高分化腺癌未见复发。A期复发率 11.8% (2 /17) ,B1期复发率 2 2 .2 % (2 /9)。结论 低位直肠癌超低位前切除适应证为 :肿瘤局限于粘膜下层 ,病理为高分化腺癌 ,肿瘤下缘距齿状线 2 .5cm以上 (自然状态 )。
Objective To explore the indications for ultra low anterior resection of rectal cancer. Methods Combined literature analysis of 26 hospitals from January 1994 to December 2000 were performed in 26 patients with low rectal cancer. Results The five-year survival rate (85.4±8.1)% (life table method). The total local recurrence rate was 15.4%, and the recurrence rate of moderately differentiated adenocarcinoma was 25% (2/8). No recurrence was found in well-differentiated adenocarcinoma. The relapse rate in stage A was 11.8% (2/17), and the relapse rate in stage B1 was 22.2% (2/9). Conclusions The indication for ultralow anterior resection of rectal cancer is: The tumor is confined to the submucosa, and the pathology is well-differentiated adenocarcinoma. The lower edge of the tumor is more than 2.5 cm away from the dentate line (natural state).
