目的根据幽门螺杆菌致上消化道癌的特点以及太行山南部上消化道癌的地理分布特征,讨论幽门螺杆菌感染与该地域上消化道癌的相关性。方法首先描述该地域上消化道癌独特的癌谱和地理分布,提示其与幽门螺杆菌感染的相关性,然后通过综述该地区开展过的幽门螺杆菌感染的研究文献以及比较平原和山区社会经济发展和饮水卫生状况的差距,进一步探讨上消化道癌分布与幽门螺杆菌感染的相关性。结果这个地区食管鳞癌、贲门及胃远端腺癌共同高发,从平原到山区胃癌发病率逐步增加,提示饮水不安全、幽门螺杆菌感染可能与上消化道癌有关。结论当前太行山南部上消化道癌高发区快速工业化刚刚开始,预计不久的将来人群幽门螺杆菌感染率及上消化道癌谱将会发生显著地改变,所以目前是开展幽门螺杆菌感染和上消化道癌谱人群监测的最佳时机。
Objective According to the characteristics of Helicobacter pylori-induced upper gastrointestinal cancer and the geographical distribution of upper digestive tract cancer in southern Taihang Mountains, the relationship between Helicobacter pylori infection and upper gastrointestinal cancer in this area was discussed. Methods Firstly, the unique cancer spectrum and geographical distribution of upper gastrointestinal cancer in this area were described, and its correlation with Helicobacter pylori infection was suggested. Then, the literature about Helicobacter pylori infection in this area was reviewed and the socio-economic comparison of plain and mountainous areas Development and drinking water health status of the gap to further explore the distribution of upper gastrointestinal cancer and Helicobacter pylori infection correlation. Results The esophageal squamous cell carcinoma, cardia and distal gastric adenocarcinoma were common in this area. The incidence of gastric cancer increased gradually from plains to mountainous areas, suggesting that drinking water was not safe. Helicobacter pylori infection may be related to upper gastrointestinal cancer. Conclusions At present, the rapid industrialization of upper digestive tract cancer in the southern part of Taihang Mountain has just begun. It is estimated that the prevalence of Helicobacter pylori and the upper digestive tract cancer spectrum in the population will change significantly in the near future. Therefore, it is currently carrying out Helicobacter pylori infection and upper gastrointestinal tract Cancer screening the best time to monitor.
