目的用三维治疗计划系统评估前列腺癌常规放射治疗中肿瘤及周围组织剂量的分布。方法未经治疗的前列腺癌患者20例经体位固定、模拟机定位、体膜上标记射野中心后进行CT扫描,通过局域网将扫描图像传送到治疗计划系统。由主治医生先行肠道和膀胱造影,然后CT勾画GTV、CTV和PTV。物理师为每例患者设计3种野:A为常规野:以体膜上显示的标记点为模拟机定位中心,将模拟机定位的条件直接输入治疗计划系统;B为扩大野;C为适形野:用三维治疗计划系统使靶区(GTV、CTV和PTV)和正常组织达到满意剂量。结果所有3种照射技术,95%等剂量线均能覆盖GTV、CTV,95%以上的PTV均能为90%等剂量线所包绕。在保护直肠方面,C适形野技术最佳。靶区剂量均匀性好,能明显减少膀胱的受照剂量,对股骨头的保护最好。结论三维适形放射疗法治疗前列腺癌疗效好、损伤轻,值得推广。
Objective To evaluate the dose distribution of tumor and surrounding tissue in routine radiotherapy of prostate cancer with three-dimensional treatment planning system. Methods Twenty patients with untreated prostate cancer underwent fixed-body positioning and simulator positioning. The CT scan was performed on the body membrane labeled with the radio field center, and the scanned images were sent to the treatment planning system through the local area network. The chief physician pre-empted gut and cystography, then CT sketches GTV, CTV and PTV. Physician for each patient to design three kinds of wild: A for the routine field: the point displayed on the body membrane as a simulation machine positioning center, the simulation machine positioning conditions directly into the treatment planning system; B to expand the field; C is appropriate Morita: A three-dimensional treatment planning system to the target area (GTV, CTV and PTV) and normal tissue to a satisfactory dose. Results All three irradiation techniques, 95% isodose lines can cover GTV, CTV, 95% of the PTV can be surrounded by 90% isodose lines. In the protection of the rectum, C conformal wild technology best. Target dose uniformity, can significantly reduce the dose of the bladder dose, the best protection of the femoral head. Conclusions Three-dimensional conformal radiotherapy for the treatment of prostate cancer has good curative effect and less damage. It is worth promoting.
