目的了解危重孕产妇基本情况,探索其相关影响因素,为降低危重孕产妇发生率提供科学依据。方法经卫生信息监测培训的专人收集入院孕产妇个案相关信息,并录入网络直报系统,调查内容包括孕产妇的个人信息、妊娠终止与合并症或并发症、抢救措施与抢救过程等。结果危重孕产妇发生率0.37%,省、部级医院发生率最高;25~34岁、高中/中专学历、多次妊娠、经产妇、单胎、足月妊娠者发生率最高;合并和/或并发以出血、贫血最多见,其次为高血压及糖尿病,分别占86.86%、68.61%、24.82%、21.90%;抢救措施上输血比例最高,占84.67%,进入ICU者占43.80%,子宫切除术占23.36%。结论加强文化层次中、低下的中青年的性教育,减少妊娠次数,控制剖宫产率,有利于降低危重孕产妇发生率;加强孕产期保健管理、宣教、追访,可减少急危重症的发生;加强基层医院急救能力,提高产科质量,完善转诊机制,加强综合性医院产科与ICU等相关科室的合作,能够提高危重症孕产妇抢救成功率。
