目的研究喉癌患者外周血CK19mRNA、CK20 mRNA的表达,探讨其临床意义。方法采用巢式RT-PCR技术检测喉癌患者术前外周血中CK19mRNA、CK20 mRNA的表达。结果48例喉癌患者外周血中CK19 mRNA阳性率为37.5%(18/48),CK20 mRNA阳性率45.8%(22/48)。10例肿瘤组织中CK19 mRNA阳性率为100%(10/10),CK20 mRNA阳性率80%(8/10);10例声带息肉患者外周血为阴性对照,CK19 mRNA阳性率为10%(1/10),CK20mRNA阳性率为0。结合临床病理资料,喉癌患者外周血CK19 mRNA、CK20 mRNA表达与喉癌T分期、临床分期及淋巴结转移存在相关性,差异有统计学意义(P<0.05)。结论采用巢式RT-PCR技术,对喉癌患者外周血单个核细胞CK19mRNA和/或CK20 mRNA的检测,在分子生物学水平对喉癌细胞对周围组织的浸润及局部扩散提供了参考,有助于指导临床治疗。
Objective To study the expression of CK19 mRNA and CK20 mRNA in peripheral blood of patients with laryngeal cancer and to explore its clinical significance. Methods Nested RT-PCR was used to detect the expression of CK19 mRNA and CK20 mRNA in peripheral blood of laryngeal cancer patients before operation. Results The positive rate of CK19 mRNA in peripheral blood of 48 laryngeal cancer patients was 37.5% (18/48) and that of CK20 mRNA was 45.8% (22/48). The positive rate of CK19 mRNA was 100% (10/10) and the positive rate of CK20 mRNA was 80% (8/10) in 10 cases. The peripheral blood of 10 patients with vocal cord polyp was negative control, the positive rate of CK19 mRNA was 10% (1 / 10), CK20 mRNA positive rate of 0. Combined with clinicopathological data, the expression of CK19 mRNA and CK20 mRNA in peripheral blood of laryngeal cancer patients were correlated with T stage, clinical stage and lymph node metastasis of laryngeal cancer, with statistical significance (P <0.05). Conclusion The detection of CK19 mRNA and / or CK20 mRNA in peripheral blood mononuclear cells of laryngeal cancer patients using nested RT-PCR technique provides reference for the infiltration and local spread of laryngeal carcinoma cells to the surrounding tissues at molecular biology level To guide clinical treatment.
