目的探讨超声萤火虫成像技术对痛风性关节炎(GA)的诊断价值。方法痛风患者GA组62例,包括急性期24例和慢性期38例。无症状高尿酸血症患者AH组22例。对两组患者行二维超声和“萤火虫”技术检查,扫描部位为第一跖趾关节和膝关节。取内侧、外侧、正中三个切面,重复扫查三次,记录检测出的强回声数目的平均值。结果 (1)两组患者超声萤火虫技术检测出的微结晶数明显高于二维超声(P<0.001);(2)GA组患者的微结晶数明显高于AH组(P<0.001);(3)第一跖趾关节的微结晶多见于内侧和正中切面,膝关节的微结晶多见于内侧和外侧切面;且前者两种技术微结晶的检出率均高于后者;(4)两种技术在GA组急性期检测出的微结晶数目比慢性期增多(P<0.05)。结论超声萤火虫技术对痛风性关节炎微结晶的灵敏度优于二维超声,对该病的诊断具有重要价值。
Objective To investigate the diagnostic value of ultrasound firefly imaging in gouty arthritis (GA). Methods 62 patients with gout in GA group, including 24 cases of acute phase and 38 cases of chronic phase. Asymptomatic hyperuricemia in AH group of 22 patients. Two groups of patients underwent two-dimensional ultrasound and “firefly ” technology examination, the scan site for the first metatarsophalangeal joint and knee joint. Take the medial, lateral, median three sections, repeat the scan three times, record the average number of strong echoes detected. Results (1) The number of micro-crystallites detected by ultrasound firefly technique was significantly higher than that of two-dimensional ultrasound in both groups (P <0.001). (2) The number of micro-crystallites in GA group was significantly higher than that in AH group 3) The first metatarsophalangeal joint micro-crystallisation more common in the medial and medial section, the micro-crystallization of the knee more common in the medial and lateral section; and the former two kinds of technology microcrystalline detection rate were higher than the latter; (4) two The number of micro-crystals detected in the acute phase of GA increased more than that in the chronic phase (P <0.05). Conclusion Ultrasound firefly technology is superior to two-dimensional ultrasound in the gingivitis micro-crystallization sensitivity, which is of great value in the diagnosis of the disease.
