12导联体表心电图上诊断慢性肺心病的阳性率约30%,而向量图阳性率可达90%左右。若能将肺心病在向量图的特异改变在心电图上有所反映,这将提高心电图对肺心病的诊断率。为探讨这一设想,我们应用常规心电图12导联加做右胸导联(VR),对正常组、单纯的肺气肿组、肺心病组进行了对照观察,报告如下: 1.资料与观察方法:3组对象均用常规心电图记录12导联,并加做V_3R至V_6R4个VR,记录的VR,将心室肌的除极波QRS波群后1/2部分的R波或V波,可以认为是右心室心向量图终末前向量或终末向量环在右胸导联轴正极侧的投影,将该波命名为续发R(secondR),与QRS波群其他波型相区别。 2.结果:见附表。
