目的比较部分和完全去腐治疗急性龋的疗效。方法计算机检索The Cochrane Library、Pub Med、EMbase、Wan Fang Data、CBM、CNKI,查找相关随机对照试验,检索时限均为从建库至2015年10月。由两位评价员独立筛选文献、提取资料和评价研究质量后,采用Rev Man 5.3软件进行Meta分析。结果最终纳入7个RCT,共计685例患者。Meta分析结果显示:对乳牙龋进行部分去腐的牙髓暴露率低于完全去腐[RR=0.13,95%C(I0.05,0.37)],在术后牙髓病变方面两种方法的差异无统计学意义[RR=0.92,95%C(I0.34,2.50)]。乳牙[RR=1.07,95%C(I0.67,1.70)]、年轻恒牙龋[RR=1.50,95%C(I0.65,3.48)]无论是采用部分去腐还是完全去腐,修复失败的差异无统计学意义。结论部分去腐法治疗乳牙、年轻恒牙急性龋的效果是肯定的,值得在临床工作中推广使用。由于纳入研究设计存在一些缺陷,还需要今后开展更多高质量,大样本的随机对照试验来验证此结论。
Objective To compare the efficacy of partial and complete decorticating in the treatment of acute caries. Methods A computer-based online search of The Cochrane Library, Pub Med, EMbase, Wan Fang Data, CBM and CNKI was conducted to search for relevant randomized controlled trials. The search period was from the database to October 2015. The two reviewers independently screened the literature, extracted the data, and evaluated the quality of the study. Meta-analysis was performed using Rev Man 5.3 software. The results eventually included 7 RCTs for a total of 685 patients. The results of Meta-analysis showed that the rate of endodontic dental pulp decontamination was lower than that of complete decolouration [RR = 0.13,95% C (I0.05,0.37)], and both methods of postoperative dental pulp disease The difference was not statistically significant [RR = 0.92,95% C (I0.34,2.50)]. Deciduous tooth [RR = 1.07,95% C (I0.67,1.70)], young permanent tooth caries [RR = 1.50,95% C (I0.65,3.48)] Whether using partial decapping or completely decadent, The difference of failure was not statistically significant. Conclusions Some of the decadent decoction treatment of deciduous teeth, young permanent teeth acute caries effect is affirmative, it is worth to promote the use of clinical work. Due to the shortcomings involved in the study design, more high-quality, large-scale randomized controlled trials are needed to validate this conclusion.
