笔者在本刊1996年第2卷第2期发表《浅谈卫生防疫执法后的档案管理》一文后,收到不少来信,商讨综合执法后的档案管理问题,现将我们的有关做法简述如下,愿与同行交流.1 综合资料1.1 办证申请 食品生产经营单位开业前必须先到市(县)卫生防疫站提出申请,填写卫生许可证申请书,内容填写要完整、清晰,特别是法人代表、主要卫生设施不得遗漏,并有主管部门意见和公章.个体经营户如无主管部门(来自外地)不方便盖章者,可用身份证复印件贴于主管部门位置代替.较大单位如食品加工厂、饭店、大型建筑业等,在经预防性卫生监督验收合格后.方可准予办证.
