目的：探究抗旋转型股骨近端髓内钉(PFNA)内固定与股骨颈骨穿针法在股骨颈骨折中的临床疗效。方法选取2010年1月—2013年10月该院收治的股骨颈骨折患者100例作为研究对象，随机分为观察组和对照组，每组各50例。其中对照组患者采取股骨颈骨穿针法治疗，观察组患者采取 PFNA 进行内固定。术后跟踪随访20个月，观察并比较二组患者的疗效。结果观察组髋关节功能恢复优良率为90%，明显高于对照组72%；观察组术后开始下地负重行走时间、骨折愈合时间与对照组比较差异具有统计学意义(P <0.05)；二组患者 Harris 髋关节评分较治疗前增加，且观察组患者的髋关节评分手术前后的增加值高于对照组；观察组患者术后并发症的总发生率为8%，明显低于对照组24%，差异具有统计学意义(P <0.05)。结论PFNA 在股骨颈骨折患者中的疗效优于经皮穿针法，术后并发症少、髋关节功能恢复好，可作为股骨折患者手术的首选内固定方法。“,”Objective To explore the clinical curative effect of femoral neck fracture of the rotary resistance type proximal femoral intramedullary nailing (PFNA)internal fixation and femoral neck bone wear stitch for the treatment of femoral fractures.Methods Totally 100 patients with femoral neck fracture who were receiving treatment in our hospital from January 2012 to October 2013 were selected as the research sub-jects,which were randomly and equally divided into observation group and control group,50 cases in each group.Patients in the control group took the femoral neck bone acupuncture treatment,while the observa-tion group received the rotary resistance type proximal femoral intramedullary nail fixation.After a follow-up for 20 months,the curative effect of patients in these two groups were observed and compared.Re-sults The hip joint function recovery rate of the observation group was 90%,significantly higher than that of the control group,72%;and the postoperative get-out-of bed with weight-bearing time of the ob-servation group and the fracture healing time were significantly different compared with control group,that was of statistical significance (P <0.05);Harris hip score in these two groups were increased after treat-ment,the added value of the observation group of patients with hip score before and after treatment was higher than that of the control group.The total incidence of patients with postoperative complications in the observation group was 8%,significantly lower than that of the control group,24%,with statistical signifi-cance(P <0.05).Conclusions The curative effect of PFNA is better than percutaneous stitch in the treat-ment of patients with femoral neck fracture with less postoperative complications and good restoration of the hip joint function,and it can be used as internal fixation method of choice for patients with femoral frac-ture surgery.
