作者应用等容血液稀释(IHD)观察24例(25眼)视网膜中央静脉阻塞与分枝静脉阻塞的治疗效果。在10天内重复用血浆和低分子右旋糖酐(分子量40000)替代400ml全血,红细胞压积由平均43.52%降至33.62%。在治疗后25眼中有视力增进者18眼,随访3个月后有24眼视力增加。IHD无严重并发症。全血粘度和红细胞沉降率在治疗前后也有明显差异。由于IHD能降低血液粘度并改善缺血视网膜的微血管循环,因此它能有效地治疗视网膜静脉阻塞。
The authors observed the treatment of central retinal vein occlusion and branch vein occlusion in 24 patients (25 eyes) with isovolemic hemodilution (IHD). 400 ml of whole blood was repeatedly replaced with plasma and low molecular weight dextran (molecular weight 40000) within 10 days, and the hematocrit decreased from an average of 43.52% to 33.62%. Among 25 eyes after treatment, there were 18 eyes with improvement of vision, and 24 eyes showed increased vision after 3 months of follow-up. IHD no serious complications. Whole blood viscosity and erythrocyte sedimentation rate were also significantly different before and after treatment. IHD can effectively treat retinal vein occlusion because it decreases blood viscosity and improves microvascular circulation in the ischemic retina.
