应用图像分析方法对114例胃粘膜异型增生标本的细胞核DNA含量及核形态参数进行了定量分析,同时进行DNA含量直方图分型。结果发现其DNA含量直方图呈整倍体型者(1、Ⅱ型)73例(64%),非整洁体型者(Ⅲ、Ⅳ型)41例(36%)。非整洁体型组的DNA相对含量、超二倍体细胞、超四倍体细胞百分数及核平均面积、周长、体积均较整倍体型组高(均P<0.05)。对其中44例病人进行了6~72个月(平均24个月)的随访,结果21例整倍体型异型增生中有1例癌变,23例非整倍体型病人中有8例癌变(P<0.01),此外,整倍体型组中有6例病变减轻,而非整倍体组中仅1例病变减轻(P<0.05)。提示细胞核DNA定量分析及倍体分型对胃癌前病变的临床监测具有实用价值,非整倍体核型的出现是细胞具有癌变潜能的重要指标。
The DNA content and nuclear morphological parameters of 114 gastric mucosal dysplasia specimens were quantitatively analyzed by image analysis method, and DNA content histogram was also typed. The results showed that the histogram of DNA content was 73 (64%) in euploid type (type 1, type Ⅱ) and 41 (36%) in type Ⅲ, Ⅳ. The relative content of DNA, the percentage of diploid cells, the percentage of super-tetraploid cells, the average nucleus area, the circumference and the volume of non-neat body type group were higher than those of the euploid group (all P <0.05). Forty-four patients were followed up for 6 to 72 months (average 24 months). One of 21 ectopic carcinogensis was cancerous, and 8 of 23 aneuploidy patients were cancerous (P < 0.01). In addition, 6 lesions in euploid group were alleviated, while only 1 lesion in aneuploidy group was alleviated (P <0.05). Prompt nuclear DNA quantitative analysis and ploidy type of gastric precancerous lesions of clinical monitoring has practical value, the emergence of aneuploidy is an important indicator of cancer cell potential.
