目的探索、评价农村开展居民代谢综合征(MS)社区综合干预的措施及其效果。方法常熟市农村居民在流行病学调查基础上,筛选出MS人群,按照入选标准分为强化干预组(n=335)和常规干预组(n=356),2组年龄、性别无显著性差异;强化组给予基于社区的强化综合干预措施,常规组仅给予基本公共卫生服务要求的随访干预,干预工作持续至2011年底,观察MS人群各项指标变化情况。结果经干预后,强化组体质指数(BMI)、收缩压(SBP)、舒张压(DBP)、甘油三酯(TG)、空腹血糖(FBG)5项指标下降,与常规组相比,差异有统计学意义(P<0.01);2组干预后腰围、总胆固醇(TC)2项指标较干预前均有所下降,高密度脂蛋白胆固醇(HDL-C)显著上升(P<0.01);强化组干预后MS各组分达标率普遍升高,常规组干预后MS各项指标达标率各有升降;干预后强化组在吸烟、饮食等生活方式上较常规组有明显改变,但在参与运动方面差异不大。结论在农村地区结合国家基本公共卫生服务工作开展以社区为基础的针对MS患者的强化综合干预,是行之有效的。
Objective To explore and evaluate the measures and their effects on the community integrated intervention of residents with metabolic syndrome (MS) in rural areas. Methods Based on the epidemiological survey, rural residents in Changshu were screened out MS population. According to the inclusion criteria, they were divided into intensive intervention group (n = 335) and routine intervention group (n = 356). There was no significant difference in age and sex between the two groups ; Intensive group was given comprehensive community-based interventions, routine group only given follow-up interventions required by basic public health services, the intervention continued until the end of 2011 to observe changes in the MS population indicators. Results After intervention, the indexes of body mass index (BMI), SBP, DBP, TG and FBG in the intensive group decreased. Compared with the conventional group, the difference was significant (P <0.01). After the intervention, the indexes of waistline and total cholesterol (TC) in two groups decreased compared with those before intervention, and the levels of HDL-C increased significantly (P <0.01) After the intervention, the compliance rate of each component of MS was generally higher than that of the control group, and the compliance rate of each index of MS group increased and decreased after the intervention of the conventional group. Conclusions It is effective to conduct community-based intensive comprehensive interventions for MS patients in rural areas in combination with the national basic public health services.
