目的用3.0 T磁共振扩散张量成像(MR-DTI)定量分析偏头痛患者中脑导水管周围灰质(PAG)、黑质(SN)、红核(RN)的微观结构改变及其与偏头痛发病的相关性。方法选取符合诊断标准偏头痛患者42例、性别年龄匹配的健康志愿者22名行T2WI、3D-TOF-MRA和DTI扫描,选择双侧PAG、SN、RN为感兴趣区,分别测量两组各感兴趣区的平均扩散率(MD)值、部分各向异性指数(FA)值,采用独立样本t检验及Spearman秩相关进行统计学分析。结果 (1)与对照组相比,偏头痛组双侧PAG区FA值降低,MD值升高,均有统计学意义;(2)双侧RN、SN的FA、MD在两组间各个区域的变化不同,右侧RN的MD值变化无统计学意义,其余各区域改变均具有统计学意义;(3)具有统计学意义的所有感兴趣区的FA值、MD值与偏头痛发病显著相关。结论 DTI技术可以探测偏头痛患者PAG的选择性改变,研究结果支持PAG是偏头痛发病的特异性结构。
Objective To quantitatively analyze the changes of microstructure of periaqueductal gray (PAG), substantia nigra (SN) and nucleus pulposus (RN) in migraine with 3.0T magnetic resonance diffusion tensor imaging (MR-DTI) The incidence of the correlation. Methods Twenty-two healthy volunteers with diagnostic criteria of migraine were included in this study. 22 T2WI, 3D-TOF-MRA and DTI scan were performed among 22 age-matched healthy volunteers. PAG, SN and RN were selected as the regions of interest. The average diffusivity (MD) value and partial anisotropy index (FA) value of the region of interest were statistically analyzed by using independent samples t-test and Spearman rank correlation. Results (1) Compared with the control group, the FA value of bilateral PAG in migraine group decreased and the MD value increased, both of which were statistically significant. (2) The FA and MD of bilateral RN and SN were significantly higher in all regions (3) The FA values and MD values in all the regions of interest with statistical significance were significantly correlated with the incidence of migraine . Conclusion DTI can detect the selective changes of PAG in patients with migraine, and the results support that PAG is a specific structure of migraine.
