目的探讨胎儿纤维连接蛋白(fFN)与宫颈长度测量在预测早产中的价值。方法选择本院门诊的出现先兆早产孕妇118例,检测阴道分泌物(fFN),同时实施B超测量宫颈长度,比较其临产的时间及孕周情况。结果 fFN对14 d内分娩的阴性和阳性预测值分别是97.6%、26.5%;fFN联合宫颈长度测量对14 d内分娩的敏感性、特异性、阴性和阳性预测值分别为100%、86%、100%、38.1%,对37周内分娩的敏感性、特异性、阴性和阳性预测值分别为70.8%、91.3%、81.0%、94.8%。结论 fFN对预测14 d内分娩有很高的阴性预测值,fFN测定联合B超测量宫颈长度可以提高37周前早产阳性预测结果。
Objective To investigate the value of fetal fibronectin (fFN) and cervical length in predicting preterm delivery. Methods 118 cases of threatened premature labor in our hospital were selected to detect the vaginal discharge (fFN). At the same time, the length of cervix was measured by B ultrasound to compare the time of labor and gestational age. Results The predictive value of fFN for negative and positive childbirth within 14 days was 97.6% and 26.5% respectively. The sensitivity, specificity, negative predictive value and positive predictive value of fFN combined with cervical length measurement within 14 days of childbirth were 100%, 86% , 100% and 38.1% respectively. The sensitivity, specificity, negative and positive predictive values for delivery within 37 weeks were 70.8%, 91.3%, 81.0% and 94.8% respectively. Conclusions fFN has a very high negative predictive value for predicting childbirth within 14 days. The combination of fFN and B-ultrasound can increase the positive predictive value of preterm labor 37 weeks earlier.
