目的:观察活动期和静止期狼疮肾炎(LN)外周血单个核细胞(PBMC)B7-1(CD80)和 B7-2(CD86)mRNA表达及其与疾病活动的关系,以探讨 B7-1和 B7-2 mRNA表达异常在 LN自身免疫发生中的作用及临床意义。方法:以正常人为对照,分离15例活动期和13例静止期的LN患者PBMC,采用半定量反转录PCR的方法,检测PB-MC的 B7-1和 B7-2 mRNA表达。B7-1和 B7-2 mRNA表达与 SLE疾病活动评分指数(SLEDAI)的相关关系用直线相关分析。结果:静止期 LN组 B7-1和 B7-2 mRNA表达与正常人对照组相比无显著性差异(P>0.05),活动期LN组B7-1mRNA有明显的上升(P<0.05),B7-2 mRNA表达增高更加显著(P<o.01)。B7-2 mRNA的增高与SLEDAI呈直线正相关关系(r=0.8641,P<0.05)。结论:活动期狼疮性肾炎存在着B7mRNA表达的异常,B7-2mRNA的增高与 SLEDAI呈直线正相关关系。
Objective: To observe the expression of B7-1 (CD80) and B7-2 (CD86) mRNA in peripheral blood mononuclear cells (PBMCs) of active and quiescent lupus nephritis (LN) and their relationship with disease activity, Abnormal expression of B7-2 mRNA in LN autoimmunity and its clinical significance. Methods: PBMC from 15 patients with LN and 13 patients with quiescent LN were isolated from healthy volunteers. The expression of B7-1 and B7-2 mRNA in PB-MC was detected by semi-quantitative reverse transcription polymerase chain reaction (RT-PCR). Correlation between B7-1 and B7-2 mRNA expression and SLE disease activity score index (SLEDAI) was analyzed by linear correlation analysis. Results: The expression of B7-1 mRNA and B7-2 mRNA in quiescent LN group was not significantly different from that in normal control group (P> 0.05). The activity of B7-1 mRNA in active LN group was significantly increased (P <0. 05), B7-2 mRNA expression increased more significantly (P <0.01). There was a linear positive correlation between B7-2 mRNA and SLEDAI (r = 0.8641, P <0.05). Conclusion: There is abnormal expression of B7mRNA in active lupus nephritis. The increase of B7-2mRNA is positively correlated with SLEDAI.
