目的评价介入栓塞术对不同原因引起的盆腔出血性疾病的治疗效果。方法收集2001年至2010年本院盆腔出血性疾病患者65例,男性6例,女性49例,其中子宫肌瘤35例、膀胱癌5例、子宫内膜癌6例、宫颈癌7例、外伤盆腔出血2例,产后大出血7例,分为子宫肌瘤组(35例);癌性出血组(18例);急性大出血组(12例),患者均采用超选择性插管,栓塞相关出血动脉,术后随访1-2年。结果 65例患者介入栓塞术后出血均完全停止,手术成功率100%,随访3月,13例患者复发,总有效率80%;子宫肌瘤出血患者35例,复发5例,有效率86%;膀胱癌患者5例,复发1例,宫颈癌患者6例,复发4例,子宫内膜癌患者7例,复发2例,癌性出血组总有效率61%;外伤出血,宫颈妊娠术后出血均无复发,产后大出血患者7例,复发一例,有效率92%。结论 1介入栓塞治疗对于盆腔出血的止血疗效肯定,副作用小,复发率低;子宫肌瘤出血组与癌性病变出血组比较相对疗效好。
Objective To evaluate the therapeutic effect of interventional embolization on pelvic hemorrhagic disease caused by different causes. Methods A total of 65 patients with pelvic hemorrhagic disease were collected from 2001 to 2010 in our hospital. There were 6 males and 49 females, including 35 cases of uterine fibroids, 5 cases of bladder cancer, 6 cases of endometrial carcinoma, 7 cases of cervical cancer and 7 cases of trauma 2 cases of pelvic hemorrhage, 7 cases of postpartum hemorrhage, divided into uterine fibroids group (35 cases); cancerous hemorrhage group (18 cases); acute bleeding group (12 cases), patients were treated with superselective catheterization, embolism-related bleeding Artery, follow-up 1-2 years. Results All the 65 patients were completely stopped after the embolization. The success rate of operation was 100%. In the follow-up period of March, 13 patients relapsed with a total effective rate of 80%. 35 patients with uterine fibroids hemorrhage and 5 patients relapsed with an effective rate of 86% ; 5 cases of bladder cancer, 1 case of recurrence, 6 cases of cervical cancer, 4 cases of recurrence, 7 cases of endometrial cancer, 2 cases of recurrence, the total effective rate of cancer bleeding was 61%; traumatic hemorrhage, cervical pregnancy after surgery No recurrence of bleeding, postpartum hemorrhage in 7 patients, one case of recurrence, the effective rate of 92%. Conclusion 1 interventional embolization for the hemostatic effect of pelvic hemorrhage sure, side effects, recurrence rate is low; uterine fibroids bleeding group and cancerous lesions hemorrhage relatively effective.
