目的分析养阴通络汤联合西药治疗糖尿病脑梗死患者的临床疗效及其对患者血流动力学的影响。方法将148例糖尿病脑梗死患者随机分为联合组和对照组,每组74例。对照组患者应用常规的西药进行治疗,联合组在对照组基础上加用养阴通络汤治疗。所有患者均治疗4周,比较两组的临床疗效及治疗前后全血黏度、全血高切相对黏度、全血低切相对黏度、血浆黏度、血细胞变形指数、红细胞压积、纤维蛋白原、三酰甘油(TG)、胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)及低密度脂蛋白胆固醇(LDL-C)的水平变化。结果联合组临床总有效率显著高于对照组(93.59%比82.05%,P<0.05);两组治疗后全血黏度、全血高切相对黏度、全血低切相对黏度、血浆黏度、血细胞变形指数、红细胞压积、纤维蛋白原及TG、TC、HDL-C及LDL-C水平均较治疗前明显改善,联合组改善较对照组更加明显,差异有统计学意义(P<0.05)。结论养阴通络汤联合西药治疗糖尿病脑梗死患者可以显著的改善患者的血流动力学指标以及血脂水平,从而有效的提高治疗效果,临床推荐应用。
Objective To analyze the clinical efficacy of Yangyin Tongluo Decoction combined with western medicine in the treatment of diabetic cerebral infarction and its effect on hemodynamics. Methods 148 cases of diabetic cerebral infarction were randomly divided into combined group and control group, 74 cases in each group. Patients in the control group were treated with conventional western medicine. The combined group was treated with Yangyin Tongluo Decoction on the basis of the control group. All patients were treated for 4 weeks. The clinical curative effect and the whole blood viscosity, the relative viscosity of whole blood high shear, the relative viscosity of whole blood low shear, the plasma viscosity, the index of hemorhocyte deformity, hematocrit, fibrinogen, (TG), cholesterol (TC), high density lipoprotein cholesterol (HDL-C) and low density lipoprotein cholesterol (LDL-C) levels. Results The total clinical effective rate in the combined group was significantly higher than that in the control group (93.59% vs. 82.05%, P <0.05). After treatment, the whole blood viscosity, the relative viscosity of whole blood high shear, the relative viscosity of whole blood low shear, plasma viscosity, Deformation index, hematocrit, fibrinogen and TG, TC, HDL-C and LDL-C levels were significantly improved than before treatment, the combination group improved more obvious than the control group, the difference was statistically significant (P <0.05). Conclusion Yangyin Tongluo Decoction combined with Western medicine in patients with cerebral infarction can significantly improve the patient’s hemodynamic parameters and blood lipid levels, thus effectively improving the therapeutic effect, the clinical recommendation and application.
