目的:探讨新生儿窒息生化指标的动态变化的意义。方法:对60例窒息新生儿(依病情分轻、中、重组)和30例正常新生儿分别在24 h内,第7天采集股静脉血,测定血中肌酸激酶心型同工酶、乳酸脱氢酶同工酶-1、血肌酐、皮质醇水平,血糖测定采用末梢血。观察窒息新生儿血生化指标的动态变化并与正常新生儿进行比较。结果:窒息组24 h内血清肌酸激酶心型同工酶、乳酸脱氢酶同工酶-1、血肌酐、皮质醇水平高于对照组(P<0.05),均随病情加重而升高,在病程第7天基本恢复正常。窒息组患儿24 h内血糖变异性大,轻、中度窒息以低血糖为主,重度窒息以高血糖为主,差异有统计学意义(P<0.05)。第7天各组血糖差异无统计学意义(P>0.05)。结论:窒息新生儿血清中肌酸激酶心型同工酶、乳酸脱氢酶同工酶-1、血肌酐、皮质醇水平及血糖变化与窒息程度有密切相关性。
Objective: To explore the significance of the dynamic changes of biochemical indexes of neonatal asphyxia. Methods: Femoral venous blood was collected on the 7th day in 60 neonates with asphyxia (mild, moderate, and severe) and 30 normal newborns respectively. Blood samples were collected for determination of creatine kinase cardiac isoenzyme, Lactate dehydrogenase isoenzyme-1, serum creatinine, cortisol levels, blood glucose determination using peripheral blood. Observe the dynamic changes of blood biochemical indexes in neonates with asphyxia and compare with normal neonates. Results: Serum levels of creatine kinase, lactate dehydrogenase isoenzyme-1, serum creatinine and cortisol in the asphyxia group were significantly higher than those in the control group within 24 h (P <0.05) , In the course of the first 7 days returned to normal. In the asphyxia group, the blood glucose variability was high within 24 h, mild and moderate asphyxia was mainly hypoglycemia, severe asphyxia was mainly hyperglycemia, the difference was statistically significant (P <0.05). On the 7th day, there was no significant difference in the blood sugar between groups (P> 0.05). Conclusion: Serum creatine kinase cardiomyocyte isoenzyme, lactate dehydrogenase isoenzyme-1, serum creatinine, cortisol level and blood glucose in asphyxia neonates are closely related to the degree of asphyxia.
