目的探讨功能锻炼对烧伤后肘关节瘢痕和活动度等的影响。方法将上肢烧伤后肘关节创面愈合的40例患者,随机分为干预组(20例)和对照组(20例),2组患者均给予早期补液防止休克、清除烧伤坏死组织、营养支持等治疗。对照组给予常规护理,干预组在对照组的基础上进行功能锻炼。比较2组患者治疗3个月后肘关节屈伸活动范围、瘢痕破损创面面积和上肢日常生活活动能力(activities of daily living,ADL)评分。结果干预组肘关节屈伸活动范围、瘢痕破溃创面面积均大于对照组(t=2.399,P=0.022;t=2.263,P=0.029);上肢ADL评分高于对照组(t=2.112,P=0.041)。结论功能锻炼可以增加上肢烧伤患者肘关节活动度,提高患者的ADL能力,对预防关节和瘢痕挛缩畸形具有重要作用。
Objective To investigate the effect of functional exercise on scar and mobility of elbow joint after burn injury. Methods 40 patients with elbow healed after upper limb burns were randomly divided into intervention group (n = 20) and control group (n = 20). Both groups were given early rehydration to prevent shock, remove necrotic tissue and nutritional support . The control group was given routine nursing intervention group on the basis of the control group for functional exercise. The range of flexion and extension of the elbow joint, the area of wounds with scar scar and the activities of daily living (ADL) scores of the upper limbs were compared between the two groups after 3 months of treatment. Results The flexion and extension range of the elbow joint in the intervention group was larger than that of the control group (t = 2.399, P = 0.022; t = 2.263, P = 0.029) 0.041). Conclusion Functional exercise can increase elbow mobility in patients with upper extremity burn and improve ADL ability of patients. It plays an important role in preventing joint and scar contracture deformity.
