例1男,52岁,突感左上腹持续性剧痛5h,伴呕吐,发病后无发热、腹泻、便血。既往患风湿性心脏病30年,心房纤颤1年。查体:T37℃,上腹肌紧张,左上腹压痛明显,反跳痛(+),腹水征(-),2次B超均(-),血、尿淀粉酶,血脂正常,疑为心脏病引发肠系膜动脉栓塞。后经CT确诊为急性胰腺炎(水肿型)?
