目的 研究肝动脉的X线解剖形态与变异 ,旨在指导肝癌的介入治疗。方法 5 0 0例肝癌动脉栓塞前常规腹腔动脉造影 ,对疑有肝动脉变异的作肠系膜上动脉或腹主动脉造影 ,分析DSA图像。结果 肝动脉典型分布 3 64例 (72 .8% ) ,肝固有动脉走行与肝总动脉呈钝角 62 .4% ,直角 13 .0 % ,锐角 2 4.6%。肝动脉变异 13 6例 (2 7.2 % ) ,其中肝总动脉变异 3 0例 (占 6.0 % ) ,无典型肝固有动脉 3 2例 (占 6.4% ) ,肝动脉异位至肠系膜上动脉 5 6例 (占 5 3 .9% ) ,异位至胃左动脉2 9例 (占 2 7.9% )。结论 熟悉肝动脉的X线解剖形态与变异 ,才能确保肝癌动脉栓塞的准确性和彻底性。
Objective To study the X-ray anatomy and variation of hepatic artery and to guide the interventional treatment of liver cancer. Methods 500 cases of hepatocellular carcinoma with conventional celiac artery angiography before arterial embolization were enrolled in the superior mesenteric artery or abdominal aorta for suspected hepatic arterial variation. DSA images were analyzed. Results The hepatic artery was typically distributed in 3 64 cases (72.8%). The hepatic artery walking showed an obtuse angle of 62.4% with the common hepatic artery, a right angle of 13.0%, and an acute angle of 2.6%. Hepatic arterial variation was 316 cases (27.2%), of which 30 cases were common hepatic artery variations (6.0%), 32 cases were typical hepatic artery without arteries (6.4%), and hepatic artery was ectopic to the superior mesenteric artery 5-6. Cases (53.9%), ectopic to the left gastric artery 29 cases (2 7.9%). Conclusions To be familiar with the X-ray anatomy and variation of hepatic artery can ensure the accuracy and thoroughness of hepatic artery embolization.
