目的探讨中西医结合治疗慢性阻塞性肺疾病(COPD)并肺部感染的临床疗效。方法选取西林县中医医院2014年6月—2016年6月收治的COPD合并肺部感染患者65例,根据治疗方法的不同分为对照组31例和观察组34例。对照组患者给予常规对症处理,观察组患者在对照组基础上给予清肺化痰汤治疗,两组患者均持续治疗10 d。比较两组患者的临床疗效,治疗前后心率、肺功能指标[第1秒用力呼气容积(FEV_1)]、血清炎性因子[C反应蛋白(CRP)、白介素6(IL-6)]及血氧饱和度、动脉血氧分压及动脉血二氧化碳分压。结果观察组患者临床总有效率高于对照组(P<0.05)。治疗前,两组患者心率、FEV_1、CRP及IL-6比较,差异无统计学意义(P>0.05);治疗后,观察组患者心率、CRP及IL-6低于对照组,FEV_1高于对照组(P<0.05)。治疗前,两组患者血氧饱和度、动脉血氧分压及动脉血二氧化碳分压比较,差异无统计学意义(P>0.05);治疗后,观察组患者血氧饱和度、动脉血氧分压高于对照组,动脉血二氧化碳分压低于对照组(P<0.05)。结论中西医结合治疗COPD合并肺部感染患者的临床疗效确切,可有效改善患者心率、肺功能及炎性因子水平,提高血氧饱和度、动脉血氧分压,降低动脉血二氧化碳分压。
Objective To explore the clinical efficacy of integrated traditional Chinese and western medicine in the treatment of chronic obstructive pulmonary disease (COPD) and pulmonary infection. Methods Sixty-five patients with COPD complicated with lung infection admitted to Xiling County Chinese Medicine Hospital from June 2014 to June 2016 were divided into control group (n = 31) and observation group (n = 34) according to the different treatment methods. Patients in the control group were given conventional symptomatic treatment. Patients in the observation group were given Qingfei Huatan Decoction on the basis of the control group, and both groups were treated for 10 days. The clinical efficacy, heart rate, pulmonary function index (FEV_1), serum inflammatory cytokines (CRP, IL-6) and blood levels before and after treatment were compared between the two groups. Oxygen saturation, arterial partial pressure of oxygen and arterial partial pressure of carbon dioxide. Results The total effective rate of observation group was higher than that of control group (P <0.05). Before treatment, heart rate, FEV_1, CRP and IL-6 in the two groups had no significant difference (P> 0.05). After treatment, heart rate, CRP and IL-6 in the observation group were lower than those in the control group Group (P <0.05). Before treatment, the two groups of patients with oxygen saturation, arterial partial pressure of oxygen and arterial partial pressure of carbon dioxide, the difference was not statistically significant (P> 0.05); after treatment, the observation group patients with oxygen saturation, arterial oxygen Pressure higher than the control group, arterial partial pressure of carbon dioxide than the control group (P <0.05). Conclusion Integrative treatment of COPD with pulmonary infection in patients with clinical curative effect is exact, can effectively improve the patient’s heart rate, lung function and inflammatory cytokines level, improve oxygen saturation, arterial partial pressure of oxygen, reduce arterial partial pressure of carbon dioxide.
