参麦注射液对老年慢性阻塞性肺病并多器官衰竭患者器官功能的保护作用

目的:探讨参麦注射液(SMI)对老年慢性阻塞性肺病(COPD)并多器官衰竭(MOF)患者器官功能的保护作用及其机制。方法将66例老年慢性阻塞性肺病并多器官衰竭患者随机分为对照组(33例)和治疗组(33例)。两组患者均给予抗感染等常规西医治疗,治疗组在常规治疗基础上加用参麦注射液60 ml加入5%葡萄糖注射液100 ml静脉滴注,1次/d,连用14 d。治疗前后分别测定两组患者血浆丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、尿素氮(BUN)、肌酐(Cr)、乳酸脱氢酶(LDH)、肌酸磷酸激酶(CK)及其同工酶(CK-MB)等生化指标和外周血中T淋巴细胞亚群(CD3+、CD4+、CD8+、CD4+/CD8+比值)、自然杀伤细胞(NK细胞)活性。结果①与治疗前比较,治疗组治疗后ALT、AST、BUN、Cr、LDH、CK、CK-MB、CD8+显著下降, CD3+、CD4+、CD4+/CD8+比值、NK细胞显著升高,且明显优于对照组(均P<0.05);②治疗组临床疗效明显高于对照组( P<0.05)。结论参麦注射液对老年慢性阻塞性肺病并多器官衰竭患者肝、肾、心有较好的保护作用,并提高临床疗效。这可能与参麦注射液能改善T淋巴细胞的免疫功能有关。“,”Objective To study the mechanism and protection of Shenmaiinjection on organ functions in patients with chronic obstructive pulmonary disease with multiple organ failure in the elderly,and to investigate its mechanism. Methods 66 patients with chronic obstructive pulmonary disease with multiple organ failure in the elderly were randomly divided into the control group(33 case) and treatment group(33 case), Conventional anti-infectious therapy,etc.were given to the two groups,and SMI 60 ml+5%glucose 100 ml intravenous drip once a day was added in the treatment group for 14 days.Before and after therapy the alanine aminotransferase (ALT),aspartate aminotransferase(AST), blood urea nitrogen (BUN),creatinine (Cr), lactate dehydrogenase (LDH),creatine kinase (CK),MB isoenzyme of creatine kinase (CK-MB)and other biochemical indexes, the activities of T lymphocyte subsets (CD3+,CD4+,CD8+,CD4+/CD8+ratio),natural kil er cell(NK cell) of peripheral blood, were detected respectively in both groups. Results Compare with the data before the treatment, the ALT, AST, BUN, Cr,LDH, CK, CK-MB, CD8+decreased and the CD3+, CD4+, CD4+/CD8+ratio , NK cellat the time points of 14 days were increased significantly in the treatment group,and they were obviously higher than those in the control group (al P<0.05);The clinical ef ect of treatment group was significantly higher than the control group(P<0.05). Conclusion Shenmaiinjection can protect liver, renal and heart functions markedly of the patients with chronic obstructive pulmonary disease with multiple organ failure in the elderly, and can increase the therapeutic efect . which is related with that Shenmaiinjection can obviously improve immunological function of T lymphocyte.
