目的回顾性分析107例全数字化乳腺X线摄影表现为边界清晰的乳腺肿块患者资料,探讨与肿块良恶性相关的影响因素,以提高诊断准确率。方法搜集术前诊断为BI-RADS 3类和BI-RADS 4A类单发的边界清晰乳腺肿块患者,所有肿块均有手术病理。回顾性分析患者一般资料和乳腺X线摄影表现,包括患者年龄、肿块发生部位、大小、密度、浅分叶、周围晕征、钙化等。运用单因素和多因素分析年龄、肿块最大直径、部位、肿块密度、浅分叶、周围晕征、钙化等因素对肿块良恶性的影响。结果乳腺X线摄影对于边界清晰乳腺肿块在鉴别良恶性方面,存在着征象交叉重叠。分析表明年龄与肿瘤良恶性有关,年龄每增加1岁,恶性危险性随之增加;肿块浅分叶和无周围晕征是肿瘤为恶性的危险因素,有浅分叶的肿块呈恶性可能性是无浅分叶肿块的6.68倍;无周围晕征的肿块呈现恶性的可能性是周围有晕征肿块的5.45倍。有无钙化在良恶性上无统计学差异,但其钙化在表现形态上有差异,恶性钙化细小,良性钙化粗大。结论 X线摄影表现为边界清晰的乳腺肿块,其良恶性与年龄、肿块浅分叶、肿块周围晕征密切相关,综合分析可提高诊断准确率。
Objective To retrospectively analyze the data of 107 patients with breast masses with clear digitalized mammography and to explore the influencing factors of benign and malignant masses in order to improve the diagnostic accuracy. Methods Preoperative diagnosis of border clear breast masses in BI-RADS type 3 and BI-RADS type 4A was performed. All the tumors were surgically resected. Retrospective analysis of the general information of patients and mammography performance, including patient age, mass site, size, density, shallow lobes, surrounding halo signs, calcification and so on. Univariate and multivariate analysis of age, the maximum diameter of the mass, site, mass density, shallow leaves, surrounding halo sign, calcification and other factors on the benign and malignant tumor. Results of mammography for clear border of breast masses in the identification of benign and malignant aspects, there are signs of overlapping. Analysis showed that the age and tumor benign and malignant, each increase in age of 1 year old, malignant risk increases; shallow lobulation and surrounding halo syndrome is a malignant tumor risk factors, with shallow lobular malignant possibility was 6.68 times the non-superficial lobes; no signs of peripheral halo signs of malignancy is 5.45 times the halo signs around the tumor. No calcification in benign and malignant no significant difference, but the calcification in the performance of morphological differences, small malignant calcification, benign calcification. Conclusion X-ray photography shows a clear boundary of the breast lumps. The benign and malignant lesions are closely related to age, shallow lobes of the lumps, and halo around the lumps. A comprehensive analysis can improve the diagnostic accuracy.
