目的对2型糖尿病患者颈动脉及下肢动脉病变采用超声诊断的价值进行分析。方法选取72例2型糖尿病患者作为研究组,另选取70例正常人作为对照组。给予两组超声检查,观察两组颈动脉和下肢动脉病变(血管壁内-中膜增厚、动脉粥样硬化斑块、管腔狭窄)情况。结果研究组患者中下肢动脉血管壁内-中膜增厚17例,占23.61%,动脉粥样硬化斑块13例,占18.06%,管腔狭窄9例,占12.50%,病变发生率为54.17%(39/72);对照组患者中血管壁内-中膜增厚6例,占8.57%,动脉粥样硬化斑块4例,占5.71%,管腔狭窄2例,占2.86%,病变发生率为17.14%(12/70);研究组下肢动脉病变发生率高于对照组,差异有统计学意义(P<0.05)。研究组患者中颈动脉血管壁内-中膜增厚19例,占26.39%,动脉粥样硬化斑块12例,占16.67%,管腔狭窄有8例,占11.11%,病变发生率为54.17%(39/72);对照组患者中血管壁内-中膜增厚6例,占8.57%,动脉粥样硬化斑块5例,占7.14%,管腔狭窄3例,占4.29%,病变发生率为20.00%(14/70);研究组颈动脉病变发生率高于对照组,差异有统计学意义(P<0.05)。结论颈动脉和下肢动脉病变对2型糖尿病患者的影响较大,采用超声诊断的价值较高,可以详细了解患者的血管病变情况,有利于早期治疗,值得临床推广。
Objective To analyze the value of ultrasonic diagnosis of carotid artery and lower extremity arterial disease in type 2 diabetic patients. Methods Totally 72 patients with type 2 diabetes mellitus were selected as study group and 70 healthy individuals as control group. The two groups were given ultrasound examination. The carotid artery and lower extremity arterial lesions (vascular intima - media thickening, atherosclerotic plaque and stenosis) were observed. Results In the study group, 17 cases (23.61%) had thickened intima-media of the lower extremity arterial wall, 13 cases had atherosclerotic plaque (18.06%), 9 had stenosis (12.50%) and the lesion rate was 54.17 % (39/72). In the control group, there were 6 cases of vascular intima-media thickening (8.57%), atherosclerotic plaque in 4 cases (5.71%), stenosis in 2 cases (2.86%), The incidence rate of lower extremity arterial disease in study group was higher than that in control group, with a significant difference (P <0.05). In the study group, 19 cases (26.39%) of carotid artery intima-media thickening, 12 cases of atherosclerotic plaque (16.67%), 8 cases of stenosis (11.11%) and lesion rate of 54.17 % (39/72). In the control group, there were 6 cases (8.57%) with vascular intima-media thickening, 5 cases with atherosclerotic plaque (7.14%), 3 cases with stenosis (4.29%), The incidence of carotid artery disease was higher in the study group than that in the control group (20.00%, 14/70). The difference was statistically significant (P <0.05). Conclusion Carotid and lower extremity arterial lesions have a greater impact on patients with type 2 diabetes mellitus, and the higher value of ultrasound diagnosis can be used to understand the patients’ vascular pathological changes in detail and be beneficial to early treatment and worthy of clinical promotion.
