患者女,84岁。因“食欲不振、消瘦4个月余”就诊,无腹痛、腹胀、腰痛和便秘等症状。体检:下腹部触及质硬包块,边缘欠清,活动欠佳。实验室检查:甲胎蛋白(AFP)为1.83 ng/ml,CA199为766.89 U/ml,癌胚抗原(CEA)为1270.49n g/ml,CA125为370.5 U/ml。CT:腹盆腔见一巨大囊实性占位,大小约20.5 cm×15.3 cm×20.7 cm,以囊性成分为主,内见多发实性分隔,下缘中部见一大小约5.7 cm×4.6 cm×5.0 cm实性肿块,边界清楚,内见多发点状或小条状钙化(图1),增强扫描实性成分
