通过主诊医师组住院医疗绩效评价的实践,探讨应用DRG组数与低风险组死亡率等经典指标评价神经专科主诊医师组绩效的可行性。分析发现由于DRG分组将相似的诊断与治疗归入同一DRG组,力求利用有限的DRG组数实现同类患者的比较,对于具体某个亚专业的主诊医师组,可能并不适用于采用DRG组数评价医疗服务范围,但对于无明确亚专业方向的主诊医师组,划定DRG组范围,评价诊疗能力依然是目前有限的较客观的评价方式。在评价主诊医师组的医疗安全方面,存在低风险组死亡率等指标失效的情况,构建死亡率指数进行评价,能够保证指标可测量。
Through the practice of hospital doctor performance evaluation in the attending physician group, the feasibility of using the classical indexes such as DRG group number and low-risk group mortality to evaluate the neurological specialist physician group performance is discussed. Analysis found that as the DRG group will be similar to the diagnosis and treatment of the same DRG group, and strive to use a limited number of DRG group to achieve the comparison of similar patients, for a specific sub-professional physician group, may not apply to the use of DRG group However, for the group of GPs with no clear sub-professional direction, the scope of the DRG group and the evaluation of the diagnosis and treatment ability are still the more limited objective evaluation methods. In assessing the medical safety of the attending physician group, there was a failure of indicators such as mortality in the low-risk group, and evaluation of the mortality index was constructed to ensure that the indicator was measurable.
