目的:探讨药物涂层球囊(DCB)在治疗股腘动脉疾病的中期疗效,以期为DCB的应用提供临床证据。方法:回顾性分析2016年7月至2017年12月广州红十字会医院心血管外科收治的98例(107条患肢)股腘动脉疾病患者的临床资料,分为DCB组(55例,62条患肢)和普通球囊(POB)组(43例,45条患肢)。术中球囊扩张后残余管腔狭窄程度超过50%,给予支架植入术。收集术后2年随访资料,采用Kaplan-Meier生存分析方法和log-rank检验描述两组间的累积血管通畅率,并比较两组患者术后3、6、12和24个月的血管通畅率。结果:术中DCB组球囊扩张后植入支架6例,POB组10例。Kaplan-Meier曲线表明,两组的累积血管通畅率差异具有统计学意义(n P=0.001)。两组术后3个月的血管通畅率差异无统计学意义(n P=0.242)。与POB组患者相比,DCB组患者在术后6、12和24个月的血管通畅率较好(n P均<0.05)。分层分析表明,对于支架内狭窄病变、股动脉病变或中度钙化病变的患者,DCB组患者术后12个月和24个月的血管通畅情况良好,与POB组相比差异有统计学意义(n P均<0.05)。n 结论:DCB相比POB在治疗股腘动脉疾病方面中期疗效较好。“,”Objective:To explore the mid-term efficacy of drug-coated balloons (DCBs) in patients with femoropopliteal artery disease and further provide clinical evidence for guiding application of DCBs.Methods:The data of 98 patients with femoropopliteal artery disease treated at Department of Cardiovascular Surgery, Guangzhou Red Cross Hospital from July 2016 to December 2017 were retrospectively analyzed. The patients were divided into two groups according to whether underwent DCB angioplasty: DCB group (n n=55, 62 limbs) and plain old balloon (POB) group (n n=43, 45 limbs). If the rate of residual luminal stenosis was more than 50% after expansion of DCB/POB, stent was implanted. After collecting 2-year postoperative follow-up data of study populations, the cumulative vascular patency rates were described by Kaplan-Meier survival analysis and log-rank test and the vascular patency rates at 3, 6, 12, 24 months after operation were compared between the two groups.n Results:There were 6 cases of stent implantation after balloon expansions in DCB group and 10 cases in POB group. The Kaplan-Meier curve showed that the cumulative vascular patency rates of the two groups were significantly different (n P=0.001). There was no statistically significant difference between the two groups for vascular patency rate 3 months after operation (n P=0.242). But compared with POB group, vascular patency rates of DCB group at 6,12 and 24 months after operation were higher (all n P<0.05). For the patients with stent stenosis, femoral artery disease or moderate calcification, vascular patency rates of DCB group at 12 and 24 months after operation were higher than those of POB group (alln P<0.05).n Conclusion:DCB has better mid-term efficacy in treating patients with femoropopliteal artery disease compared with POB.
