目的:探讨细胞核增殖指数(Ki-67)、丝氨酸/苏氨酸蛋白激酶(pAKT)在胃癌组织中的表达及与胃癌侵袭转移和复发等生物学行为的关系。方法:收集有完整临床病理资料、经手术及术后病理学确诊的60例胃癌患者作为观察组,并以20例癌旁异型增生胃黏膜组织和20例活检正常胃黏膜组织作为对照组,应用免疫组织化学SP法检测Ki-67、pAKT的表达,用CD31标记血管计数MVD,比较:1)Ki-67、pAKT的表达在胃癌、癌旁组织和正常胃黏膜组织中的表达差异;2)与肿瘤分化程度的关系;3)与癌组织浸润深度和转移的关系。结果:观察组中Ki-67(73.3%,44/60)、pAKT(75.0%,48/64)的表达明显高于对照组癌旁异型组织(35.0%,7/20)和正常黏膜组织(10.0%,2/20),差异有统计学意义,χ2=9.538,P=0.002;χ2=24.621,P=0.000。Ki-67、pAKT在高级别与低级别胃癌组织中的表达差异有统计学意义,P<0.05。Ki-67、pAKT在进展期胃癌显著上调,与早期癌表达差异有统计学意义,P<0.05。Ki-67、pAKT与癌组织淋巴结转移有关,P<0.05。结论:Ki-67、pAKT异常表达与胃癌的发生、发展密切相关,在胃癌组织中表达上调,促进了肿瘤形成、发展、侵袭和转移,Ki-67、pAKT检测对于判断胃癌生物学行为有一定的价值,可作为临床参考指标。
OBJECTIVE: To investigate the relationship between the expression of Ki-67 and pAKT in gastric carcinoma and their biological behaviors such as invasion, metastasis and recurrence. Methods: Sixty patients with gastric cancer diagnosed by operation and postoperative pathology were collected as observation group. Twenty patients with gastric cancer with adjacent tissues and 20 patients with normal gastric mucosa were selected as control group. The expression of Ki-67 and pAKT was detected by immunohistochemical SP method, and the MVD was marked by CD31. 1) The expression of Ki-67 and pAKT in gastric cancer tissues, paracancerous tissues and normal gastric mucosa tissues; 2) And the degree of tumor differentiation; 3) with the depth of invasion and metastasis of cancer. Results: The expression of Ki-67 (73.3%, 44/60) and pAKT (75.0%, 48/64) in observation group was significantly higher than that in control group (35.0%, 7/20) and normal mucosa 10.0%, 2/20), the difference was statistically significant, χ2 = 9.538, P = 0.002; χ2 = 24.621, P = 0.000. The expressions of Ki-67 and pAKT in high-grade and low-grade gastric cancer tissues were significantly different (P <0.05). Ki-67 and pAKT were significantly up-regulated in advanced gastric cancer, with significant difference from early stage cancer (P <0.05). Ki-67, pAKT and lymph node metastasis, P <0.05. Conclusions: Aberrant expression of Ki-67 and pAKT is closely related to the occurrence and development of gastric cancer. Up-regulated expression of Ki-67 and pAKT promotes the formation, development, invasion and metastasis of gastric cancer. The detection of Ki-67 and pAKT may be useful in judging the biological behavior of gastric cancer The value of, as a clinical reference index.
