目的观察氩激光治疗糖尿病性视网膜病变(Panretinal photocoagulation,PRP)的临床疗效。方法氩激光治疗增殖前期(Preproliferative diabetic retinopathy,PPDR)(50只眼)及增殖期糖尿病性视网膜病变(proliferatic diabetic retinopathy PDR)(68只眼)。术前行视力、裂隙灯、眼底、视野、眼底血管造影及眼底彩色照像等检查,术后1周、1个月定期复诊,6个月左右复查视野、眼底荧光血管造影检查(Fundus fluorescence angiography,FFA),根据新生血管及无灌注区情况补充激光治疗。随诊观察3~24个月(平均10.2个月)。结果视力60只眼视力提高,占51.0%;42只眼无明显变化,占36%;视力下降者12只眼,占11%。其中4只眼激光治疗过程中出现玻璃体出血,经保守治疗后,2只眼补打激光治疗,2只眼行玻璃体切割术。经视野检查,部分视敏度略下降,无明显缩小及中心暗点出现。结论氩激光治疗糖尿病性视网膜病变安全有效,可作为一线激光。
Objective To observe the clinical efficacy of argon laser in the treatment of diabetic retinopathy (PRP). Methods Preproliferative diabetic retinopathy (PPDR) (50 eyes) and proliferative diabetic retinopathy (PDR) were performed in 68 eyes. Preoperative visual acuity, slit lamp, fundus, visual field, fundus angiography and fundus color photography and other tests, 1 week after surgery, 1 month regular follow-up, 6 months review field, Fundus fluorescein angiography , FFA), according to the new blood vessels and no perfusion area supplement laser treatment. Follow-up observation of 3 to 24 months (average 10.2 months). Results The visual acuity increased in 60 eyes, accounting for 51.0%. There was no significant change in 42 eyes, accounting for 36%. Twelve eyes had decreased vision in 11 eyes. Vitreous hemorrhage occurred in 4 eyes during laser treatment. After conservative treatment, laparotomy was performed in 2 eyes and vitrectomy in 2 eyes. Visual field examination, some visual acuity decreased slightly, no significant reduction and dark spots appear. Conclusion Argon laser treatment of diabetic retinopathy is safe and effective, can be used as a first-line laser.
