目的探讨低血钾对房室阻滞的影响。方法选择武汉同济医院近十年来7例低血钾致房室阻滞的病例,分析并总结低血钾致房室阻滞可能的原因及危害。结果经静脉及口服补钾治疗后(慢性失钾者同时补镁),除1例因重症有机磷中毒死于肾功能衰竭外,其余病例均在数小时至两天内阻滞程度有所减轻,甚至心电图恢复正常,复查血钾亦在正常范围。结论低钾所致的房室阻滞是可逆的,主要影响心肌细胞的电生理功能,并未造成心脏特殊传导系统的病理性损害,及时适量补钾可以改善低钾引起的房室阻滞。
Objective To investigate the effect of hypokalemia on atrioventricular block. Methods Totally 7 cases of hypokalemia induced atrioventricular block in Tongji Hospital of Wuhan City in the past ten years were analyzed and the possible causes and harms of hypokalemia induced atrioventricular block were analyzed and summarized. The results of intravenous and oral potassium treatment (chronic loss of potassium at the same time fill magnesium), except for 1 case of severe organophosphate poisoning died of renal failure, the remaining cases were in a few hours to two days the degree of block has been reduced, Even electrocardiogram returned to normal, the review of serum potassium is also in the normal range. Conclusions Atrioventricular block induced by hypokalemia is reversible, mainly affecting the electrophysiological function of cardiomyocytes. It does not cause pathological damage to the special conduction system of the heart. Appropriate potassium supplementation may improve the block of atrioventricular block induced by hypokalemia.
